چگونه چالش‌هاي پياده‌سازي پرونده‌ي الكترونيك سلامت را هموار كنيم؟: يك مطالعه‌ي كيفي در اصفهان

How the Challenges Of Implementing Electronic Health Records will Pave?: a Qualitative Study in Isfahan

مقدمه: دورنماي پرونده‌س الكترونيك سلامت (Electronic Health Record) در جوامع مختلف ثابت نبوده و در عين به همراه داشتن مزايا و فرصت ها، با چالش روبرو است. در اين پژوهش سعي شده چالش ها معرفي و اجراي EHR از ديدگاه متوليان آن در استان اصفهان بررسي شود. روش بررسي: پژوهش حاضر مطالعه‌اي كيفي است كه در سال 1389 خورشيدي در استان اصفهان انجام شد. با استفاده از روش پديدارشناسي (فنومنولوژي) و از طريق مصاحبه‌ي عميق نيمه ساختاريافته به استنتاج تجارب صاحب‌نظران حوزه‌ي سلامت در رابطه با چالش هاي اجراي پرونده‌ي الكترونيك سلامت و راهكارهاي حل چالش ها پرداخته شد. مصاحبه شوندگان بر اساس نمونه‌گيري به روش گلوله برفي (Snow Ball) مشتمل بر 3 پزشك، 4 نفر از مديران بخش هاي مختلف سلامت و 8 نفر از ساير متخصصاني بودند كه در سطح بيمارستان‌ها و ساير مراجع مرتبط در اين زمينه مانند معاونت درمان، سازمان‌هاي بيمه، ستاد دانشگاه و استانداري در رابطه با محتوا و ساختار پرونده‌ي الكترونيك سلامت مطلع و در اين زمينه داراي فعاليت‌هاي پژوهشي بوده و يا در برنامه‌ريزي و معرفي پرونده‌ي الكترونيك سلامت در استان اصفهان فعاليت داشتند. يافته‌ها: پژوهشگر چالش‌هاي پرونده‌ي ‌الكترونيك سلامت در استان اصفهان و راهكارهاي مربوط به حل آن‌ها را در دو بعد زيرساختاري و ساختاري استخراج و تبيين كرد. نتيجه‌گيري: متاسفانه نتايج اين مطالعه نشان داد با وجود آن‌كه پزشكان از مهم‌ترين كاربران پرونده‌ي الكترونيك سلامت‌اند، اما كم‌ترين نظرات را در رابطه با چالش‌ها ارايه داده‌اند و اين كاستي قابل تامل است چرا كه اگر دال بر كم‌رنگ بودن نقش آن‌ها در معرفي و اجراي پرونده‌ي الكترونيك سلامت باشد اين امر نتايج زيان‌باري به دنبال خواهد داشت. واژه هاي كليدي:پرونده ي الكترونيك سلامت؛چالش ها؛راهكارها

Abstract Introduction: Perspective of Electronic Health Records (EHR) is not constant in different societies and facing with the opportunities and challenges. In this study attempted to identify challenges of EHR implementation of point of view of custodians in Isfahan Province. Methods: this research was a qualitative study that conducted in 2011 in Isfahan. The method of phenomenology was used so peopleʹs experiences through semi-structured depth interview about electronic health records as a new subject in the field of health has been studied. The study population included physicians (n=3), administrators (n=4) and experts in different fields (n=8) with at least a masterʹs degree is related to the content and structure of electronic health records and inform research activities in this area have been or in the planning and introduction of electronic health records in the role of have contributed. Information obtained through hospitals and other relevant authorities in this field such as Deputy Treatment, insurance agencies, universities and General Staff were collected. Using the snowball sampling method that have first inclusion criteria were interviewed, because they then other people who possess the qualifications for entering the study were identified and referred to them for the interview was done. This continuum in order to reach saturation phase information (Information Saturation) and thus continued to study sample consisted of 15 people three doctors, four people from different parts of health managers and eight were experts in the field of electronic health records reached. Results: The researcher divided the challenges to the two areas of infrastructure and structural. Conclusion: Unfortunately, the results of this study showed although physicians are the most important users of electronic health records but at least the comments have presented. If the evidence is due to weakness of their role in the introduction and implementation of electronic health records, these disastrous results will follow.
