ارزيابي فرايند اولويت‌گذاري بر اساس چارچوب پاسخگويي براي منطقي بودن در دانشگاه علوم پزشكي كرمان

Evaluation of Priority Setting Process Based on Accountability Framework for the Rationality in Kerman University of Medical Science

مقدمه: با توجه به محدوديت منابع و نيازهاي نامحدود به خصوص در زمينه خدمات بهداشتي و درماني و نقش مهم بهداشت و درمان در سلامت افراد جامعه مساله تخصيص منابع محدود به نيازهاي نامحدود در حوزه خطير بهداشت و درمان حايز اهميت مي‌باشد. از همين رو اين مطالعه با هدف ارزيابي اولويت‌گذاري بر اساس چارچوب پاسخگويي براي منطقي بودن در دانشگاه علوم پزشكي كرمان انجام گرفته تا فرايند اولويت‌گذاري را مورد شناسايي قرار دهد. روش بررسي: پژوهش حاضر مطالعه‌اي كيفي است كه از طريق مصاحبه با 18 نفر از صاحب‌نظران سلامت در خصوص تخصيص منابع داشگاه علوم پزشكي كرمان 6 ماه اول سال 1391 صورت گرفته است. براي انجام پژوهش چارچوب استانداردDaniels & Subin مورد استفاده قرار گرفته است. ابزار گردآوري داده‌ها، مصاحبه نيمه‌ساختار‌يافته بود. جهت قابليت انتقال يافته‌ها سعي گرديد نمونه غير متجانسي از افراد درگير در خصوص تقسيم بودجه در معاونت‌ها استفاده شود .جهت اطمينان‌پذيري و تاييدپذيري متن نسخه‌برداري شد و تجزيه و تحليل حاصل از آن به استادان مشاركت‌كننده در اين پژوهش ارايه گرديد و پيشنهادهاي اصلاحي و نظرات ايشان اعمال شد. تجزيه و تحليل داده‌ها بر اساس روش تحليل چارچوبي صورت گرفته است. يافته‌ها: طي اين مطالعه 4 كد اصلي شامل شفافيت، منطقي بودن، كنترل و نظارت، بازنگري و 9 كد فرعي شامل قابل پيش‌بيني بودن، ابهام، منطقي بودن، عدم توزيع منطقي، دو طرفه بودن، يك طرفه بودن، تناسب ميان ساز و كار و منابع، وجود تجديدن نظر، عدم وجود سازوكار مي‌باشند. نتيجه‌گيري: فرايند اولويت‌گذاري در دانشگاه علوم پزشكي كرمان بر اساس چارچوب دانيلز و سابين از منطق اندكي برخوردار است. تصميم‌گيري بدون بهره‌گيري از روش‌هاي دقيق و مشخص اندازه‌گيري هزينه‌ها و منافع مداخلات مختلف و مبتني ساختن تصميم‌گيري‌ها بر نتايج اين بررسي‌ها، منجر به استفاده بهينه از منابع محدود نخواهد گرديد. واژه هاي كليدي: اولويت هاي بهداشتي؛ منطقي بودن؛ تخصيص منابع؛ دانشگاه ها

Abstract Introduction: According to resource constraints and unlimited needs, health care has important role in the public health. The allocation of limited resources to unlimited needs in critical areas of health is important. Therefore, the purpose of this study is to evaluate the process of priority setting based on accountability framework for the rationality in Kerman university of Medical Science. Methods: This research is a qualitative study. 18 people from health experts about resource allocation in Kerman University of Medical Science interviewed. In this study Daniels and Sabin framework was used. Data analysis techniques were based on the analysis framework. Tools of data gathering were Semistructured interviews. Heterogeneous sample of the people involved in the division of the departmentʹs budget is used to transfer capability. In order to credibility and confirm ability text transcribed and analysis of the resulting professors participating in this study were presented and corrective recommendations their comments were applied. Results: The study includes four main codes: transparency, rationality, control and monitoring, overview and 9 sub-codes contains: Predictability, ambiguity, rationality, no sensible distributor, bilateral, unilateral, Proportionality between the mechanism and resources, revision, no mechanism. Conclusion: The status indicates the process of priority settings in Kerman University of Medical Science according to the Daniels and Sabin has little logic. No specific decision procedures, measuring the costs and benefits of different interventions so, making decisions based on the results of these studies won’t lead to optimal use of scarce resources.
