Developing Intelligent Electronic Triage System Using the Emergency Severity Index

مقدمه: با توجه به گسترش روزافزون كاربرد رايانه در حوزه بهداشت و درمان و ارايه مدل هاي ترياژ الكترونيك در برخي كشورها و با عنايت به توصيه سيستم ترياژ ESI -Emergency Severity Index به عنوان مدل مرجع ترياژ توسط وزارت بهداشت درمان و آموزش پزشكي، اين مطالعه با هدف طراحي مدل بومي كشوري ترياژ الكترونيك انجام گرديد. روش بررسي: سامانه هوشمند ترياژ الكترونيك بر مبناي ويرايش چهارم دستنامه ترياژ ESI به صورت تحت وب و با زبان برنامه نويسي PHP و پايگاه داده MySQL طراحي گرديد. هدف از اتخاذ اين ساختار، دقت، سرعت و سهولت انجام فرآيند ترياژ بطريقه الكترونيك بود. اطلاعاتي كه در اين سامانه ثبت مي شود شامل اطلاعات دموگرافيك و اطلاعات باليني است كه بر اساس آن سطح ترياژ و محل درمان بيمار تعيين مي گردد. يافته ها: اين سامانه به منظور استفاده در بخش اورژانس بيمارستان طراحي شد. كاربران اصلي آن پرستاران ترياژ، پزشكان اورژانس و سوپروايزرها و مسيولين شيفت اورژانس هستند. علاوه بر انجام ترياژ به صورت هوشمند، قابليت جستجو، گزارش گيري و محاسبه شاخص هاي مهم وزارت بهداشت در مورد ترياژ –ميانگين زمان ترياژ و ميزان ترياژ صحيح- از مزاياي قابل توجه اين سامانه مي باشند. نتيجه گيري: طي اين مطالعه سامانه بومي ترياژ الكترونيك براي اولين بار در كشور در دانشگاه علوم پزشكي تبريز طراحي شد. با توجه به رويكرد بيمارستان ها در استفاده از سيستم هاي رايانه اي، مدل ترياژ الكترونيك مي تواند بعنوان الگويي براي بهره گيري از رايانه در بخش هاي اورژانس بيمارستان ها باشد. واژه هاي كليدي: خدمات اورژانس بيمارستان؛ ترياژ؛ ترياژ الكترونيك

Abstract Introduction: With regards to the growing use of computers in the triage along with other parts of healthcare systems, and by considering this fact that the Ministry of Health and Medical Education of Iran suggests the Emergency Severity Index (ESI) as an appropriate triage system, this study was conducted to develop a domestic model for an electronic triage system. Methods: The intelligent Electronic Triage System (ETS) was developed based on ESI version4, using PHP programming language and MySQL data base. The system designed to be a web based application. The purpose of this structure was to improve the triage process in terms of precision, speed and ease of use. The ETS, based on input data, determines triage level and treatment room automatically. Results: The ETS was designed to be used in Emergency Departments (EDs) in hospitals. Its main users are the triage nurses, ED physicians, ED head nurses and supervisors. Besides performing triages automatically, the ETS has some advantages such as search and report functions, and also calculating two indexes suggested by the Ministry of Health and Medical Education on triage, including average triage time and the under-triage and over-triage rate. Conclusion: In this study the domestic Electronic Triage System was developed in Tabriz University of Medical Sciences, for the first time in Iran. With current trend of implementing computerized systems in hospitals, the Electronic Triage System can be used as a model for computerizing triage at EDs.
