بررسي تأثير روش درماني پاسخ محور بر توانايي آغازگري كودكان اوتيستيك

Effect of pivotal response treatment method on initiation skill in children with autism

هدف: اوتيسم يكی از اختلالات فراگير رشدی است كه تعامل اجتماعی، ارتباط كلامی و غيركلامی و الگوهای رفتاری كليشه ای را تحت تأثير قرار می دهد. با توجه به مشكلات آغازگری اين كودكان و ضعف مداخلات در ايجاد انگيزه، در اين مطالعه سعی كرديم تأثير روش درمانی پاسخ محور با كاربرد انگيزه را بر توانايی آغازگری كودكان اوتيستيك بررسی نماييم. روش بررسی: در اين مداخله كارآزمايی بالينی با گروه كنترل، طبق معيارهای ورود و خروج، 30 فرد اوتيستيك 11-6 ساله كه دو به دو از نظر سن و جنس و ميانگين طول گفته همتا بودند از كلينيك های مخصوص افراد اوتيستيك شهر همدان انتخاب و بطور تصادفی در دو گروه مداخله و كنترل قرار گرفتند. جلسات بصورت فردی در طی 3 ماه(هفته ای 2 جلسه 60 دقيقه ای) برگزار شد. افراد هر دو گروه قبل و بعد از مداخله با استفاده از پرسشنامه مهارت های ارتباطی كودكان برای ارزيابی توانايی آغازگری ارزيابی شدند. تجزيه و تحليل داده ها با استفاده از SPSS نسخه 18 توسط آزمون های آمار توصيفی و آزمون كوواريانس انجام يافت. يافته ها: يافته های مطالعه نشان داد كه طبق آزمون كوواريانس بين دو گروه بعد از مداخله، تغييرات معناداری در توانايی آغازگری(001/0>P ) در گروه مداخله وجود دارد. نتيجه گيری: يافته های اين پژوهش نشان دهنده ی تأثير روش درمانی پاسخ محور برتوانايی آغازگری كودكان اوتيستيك می باشد و بهتر است در مداخلات درمانی اين روش درمانی نيز بكار برده شود.

Introduction: : Autism is a pervasive developmental disability that impress on social interaction, verbal and nonverbal communication and stereotyped behavioral patterns. Regarding initiation difficulties of these children and weakness of interventions in creation of motivation,in this study we tried to examine effect pivotal response treatment method on initiationʹs ability of autistic children. Material & Methods: In this randomized control trial (RCT) study, according to inclusion and exclusion criteria, 30 autistic participants (6 to 11 years old) that matched based on age, gender and mean length utterance, from especial clinics of Hamedan’s autistic persons selected and placed randomly in one of groups of intervention and control. Sessions was administered individually, lasting for 60 minutes and 2 days a week for 3 months. Both groups of children were tested before and after intervention by CCC (children’s communication checklist) to assess initiation’s ability. Data analysis was done through descriptive statistics tests and covariance analysis. Results: Findings of study showed (according to covariance analysis between before and after of intervention in two groups) there is significant changes in initiation’s ability in intervention group. Conclusion: the result of these study indicated effect of pivotal response treatment on initiation’s ability in autistic children, therefore this method recommended for speech and language pathologist.
