بررسي اثر بخشي گاباپنتين خوراكي بر شدت درد بيماران مبتلا به كمردرد حاد

Evaluation of the therapeutic effect of oral gabapentin on the severity of acute low back pain

چكيده زمينه و هدف:كمردردحاد يكي از شايع ترين عللمراجعهبيمارانبهپزشكاست كه امروزه داروهاي مختلفي براي درمان آن پيشنهاد مي شود. انتخاب داروي مناسب با اثر بخشي بالا و حداقل عوارض هميشه يك موضوع چالش برانگيز دردرمان هاي پزشكي مي باشد. مطالعه حاضر به منظور بررسي اثر بخشي گاباپنتينخوراكيبرشدتدردبيمارانمبتلابهكمردردحاد صورت گرفت. روش بررسي: اين مطالعهكارآزماييبالينيدوسوكور بر روي 100 بيمار مبتلا به كمردرد حاد مراجعه كننده به مراكز درماني وابسته به دانشگاه علوم پزشكي شهركرد طي سال 1391(فروردين الي اسفند ماه) انجام شد. بيماران پس از انتخاب به شيوه نمونه گيري تصادفي ساده در دو گروه مداخله(تعداد=50) و كنترل(تعداد=50) قرار گرفتند.براي گروه مداخله روزانه دو قرص ناپروكسن500 ميلي گرمي و دو كپسول گاباپنتين100 ميلي گرمي و براي گروه كنترل روزانه دو قرص ناپروكسن 500 ميلي گرمي و دو كپسول پلاسبو تجويز شد. بيماران در روزهاي صفر، 8، 15و 30 از نظر شدت درد بر اساس شاخص VAS و ميزان بروز عوارض جانبي ارزيابي شدند. تجزيه تحليل داده ها با نرم افزار SPSS و استفاده از آزمون هاي آماري توصيفي و استنباطي انجام شد. يافته ها: در اين مطالعه ميانگين نمره درد در معاينه روزهاي8، 15و 30 در گروه مداخله به طور معناداري كمتر از گروه كنترل بود (05/0 > p). همچنين در خصوص مقايسه درد در دو گروه مداخله و كنترل بر حسب جنس، در زنان گروه مداخله ميانگبن نمره درد در هر 3 معاينه به طور معناداري كمتر از گروه كنترل بود ولي در مردان گروه مداخله و كنترل تفاوت معناداري در شدت درد ديده نشد. نتيجه گيري: در اين مطالعه درد در بيماران گروه مداخله در مقايسه با گروه كنترل به طور قابل توجهي كاهش يافته بود. چنين استنباط مي شود تاثير گاباپنتين همراه با داروي ضدالتهابي غيراستروييدي(NSAIDs) بيشتر از NSAID ها به تنهايي است. كليد واژه‌ها: گاباپنتين، شدت درد، كمردرد حاد.

ABSTRACT Background and Aim: Acute low back pain is one of the most commoncomplaintsof the patients seeking medical advice. Nowadays various drugs are recommended for the treatment of this problem. Selection of appropriate medications with high efficacy and minimal side effects has always been a challenging issue in medical treatments. The present study was conducted to investigate the effect of oral gabapantin on pain intensity in patients with acute low back pain. Material and Methods: This double-blind clinical trial study included 100 patients with acute low back pain who had referred to the treatment centers affiliated with Shahrekord University of Medical Sciences in 2011-2012.Using convenient sampling method the patients were randomly assigned to intervention (n=50) and control (n=50) groups. Intervention group received two 500 mg naproxen tablets and two100 mg gabapantin tablets/day and control group, received two 500 mg naproxen tabletsand two placebo tablets/ day. The patients were examined for pain intensity based on visual analogue scale and the rate of complications on the days of 0, 8, 15, and 30.Using SPSS software, data analysis was performed by descriptive and inferential statistics. Result: In this study, mean pain score was significantly lower in the intervention group compared to the control group (p < 0.05). Also, the women in the intervention group had a lower mean pain score on all 3 examinationscompared to the women in the control group.But no statistically significant difference was observed in pain intensity between the men in the intervention and control groups. Conclusion: In this study, pain significantly relieved in the patients in the intervention group compared to those in the control group. It seems that use of gabapantin in addition to nonsteroidal anti-inflammatory drugs (NSAIDs) is more effective than NSAIDs alone in reducing the pain intensity.
