مقايسه ميزان جابجايي كانال ريشه مزيو باكال مرلر اول فك بالا به دنبال آماده سازي با استفاده از فايل هاي استنلس استيل به دو روش دستي و چرخشي

Comparison of canal displacement rate of mesiobuccal root of the maxillary first molar after the preparation by two manual and rotating methods using stainless steel hand files

زمينه و هدف: ابزارهای گوناگونی برای كاهش زمان آماده سازی كانال معرفی شده است. يكی از اين ابزار هندپيس نوين (TEP-E10R) است كه به نام اندوگريپر معروف است. اين مطالعه با هدف مقايسه ميزان تغيير مركزيت كانال ريشه مزيوباكال مولر اول فك بالا به دنبال آماده سازی به دو روش دستی و چرخشی با استفاده از فايل های دستی استنلس استيل انجام شده است. روش بررسی: در اين مطالعه تجربی آزمايشگاهی 50 دندان كشيده شده مولر اول ماگزيلای انسانی انتخاب و به دو گروه تقسيم شدند. بعد از تهيه حفره دسترسی و قرار دادن فايل اوليه داخل كانال مزيو باكال، راديو گرافی های ديجيتالی به صورتی كه رابطه فيلم و زاويه تابش با نمونه ها ثابت باشند تهيه گرديد، سپس آماده سازی كانال ها در گروه اول با فايل های استنس استيل به روش دستی و در گروه دو با فايل های استنس استيل با اندوگريپر انجام شد. تكنيك آماده سازی كانال هادر هر دو گروه passive step back بود. پس از آماده سازی، راديوگرافی با فايل مستر اپيكال با همان شرايط راديوگرافی اول انجام گرفت. ميزان تغيير مركزيت كانال راديو گرافی اوليه و راديوگرافی با فايل اصلی بر اساس تصاوير انطباقی بر حسب ميلی متر توسط نرم افزار فتوشاپ و به كمك راديولوژيست محاسبه شد. يافته ها: ميانگين انحراف از مسير اصلی كانال در گروه اول (36/1±45/3 ميليمتر) بيشتر از گروه دوم (69/1±75/1 ميليمتر) بود (045/0=P). همچنين ميانگين زمان آماده سازی كانال ها در گروه اول (53/1±36/19 دقيقه) بيشتر از گروه دوم (55/1±03/12 دقيقه) بود (0001/0=P). نتيجه گيری: استفاده از فايل های استنلس استيل با هندپيس (رفت و برگشتی) reciprocal نسبت به استفاده از فايل های استنلس استيل به تنهايی مسير اصلی كانال را بهتر حفظ می كند.

Background and aims: Various devices have been offered to reduce the time of channel preparation. One of these devices is a modern handpiece (TEP-E10R) which is known as endogripper. This study was conducted to determine canal displacement rate of mesiobuccal root of the maxillary first molar after the preparation by two manual and rotating methods using stainless steel hand files. Methods: In this laboratory experimental study, 50 teeth taken out from human maxillary first molar were selected and divided into two groups. After access cavity preparation and placement of original files in mesiobuccal canal, digital radiography was done as the relationship between film and emission angle, and samples was fixed. Then, canals were prepared using stainless steel manually in group one and by endogripper in group two. The technique for preparing canals in both groups was passive step back in both groups. After preparation, radiography was done using master apical file as the first radiography was done. Canal displacement rate of primary radiography and original file-assisted radiography was calculated in millimeter (mm) according to adaptive images by photoshop software and radiologist assistance. The data were compared by t-test. Results: Mean deviation from original canal route in group one was 3.45±1.36 mm, higher than 1.75±1.69 obtained in group two (P=0.045). Also, mean time for canal preparation in group one was 19.36±1.53 min, higher than 12.03±1.55 min in group two (P=0.0001). Conclusion: Using stainless steel files with reciprocal handpiece keeps on original canal route better compared to stainless steel files alone.
