ارزيابي والدين از دسترسي و موانع دسترسي به خدمات سلامت دهان و دندان در كودكان و نوجوانان در شهر اصفهان

Access to dental care and its barriers in 6‒14-year-old children reported by their parents in Isfahan

چكيده مقدمه: دسترسي و توزيع يكسان خدمات بين همه‌ي افراد، به عنوان يكي از اهداف اصلي سرويس‌هاي سلامت هر جامعه‌اي است و رضايتمندي بيمار فاكتور مهمي در ارزيابي اين اهداف مي‌باشد.اين مطالعه با هدف بررسي دسترسي به خدمات دنداني و نيز رضايتمندي از اين خدمات از ديدگاه والدين كودكان 14-6 ساله شهر اصفهان در سال 1390 صورت گرفت. مواد و روش‌ها: اين مطالعه‌ي توصيفي بر روي 390 نفر از والدين كودكان 14-6 ساله‌ي ساكن شهر اصفهان واز طريق پرسش‌نامه و مصاحبه صورت گرفت. محتواي پرسش‌نامه شامل خصوصيات دموگرافيك، بررسي كلي وضعيت سلامت دهاني كودكان، اطلاعات كلي مرتبط با وضعيت رفتارهاي سلامت، شاخص‌هاي دسترسي به خدمات، موانع مربوط به دسترسي و ارزيابي ميزان رضايتمندي از اين گونه خدمات بود. داده‌ها در نرم افزار SPSS توسط روش‌هاي آماري توصيفي وآناليز رگرسيون خطي تحليل گرديد(05/0 =?). يافته‌ها: طبق گزارش والدين در اين مطالعه 78% از كودكان دسترسي به خدمات سلامت دهان و دندان داشتند، 1/22% دچار موانع بوده‌اند و شايع‌ترين موانع هزينه‌ي بالاي درمان، عدم پوشش بيمه و ترس فرزندان از دندانپزشكي بود. 48% از افراد از دسترسي به خدمات سلامت دهان راضي بودند. علت آخرين مراجعه به دندانپزشكي در 68% كودكان مشكلات مربوط به لثه و دندان و 22% به علت معاينات منظم دنداني ارزيابي گرديد. نتيجه‌گيري: طبق اين مطالعه دسترسي به خدمات دندانپزشكي از ديدگاه والدين بالاي 70% بوده و مشكل اصلي والدين هزينه‌هاي بالاي درمان، عدم پوشش بيمه‌ها و ترس از دندانپزشكي فرزندان مي‌باشد. نيمي از والدين از خدمات دريافت شده توسط فرزندشان رضايت دارند. كليد واژه‌ها: مراقبت‌هاي دهاني دنداني، رضايتمندي بيمار، دسترسي به خدمات سلامت

Abstract Introduction: Access to and equal distribution of healthcare services are one of the main goals of any public health service and the patient satisfaction is an important factor in evaluating these goals. The purpose of this study was to assess barriers to the accessibility of and the satisfaction of parents of 6?14-year-old children with these services in Isfahan in 2011. Materials and methods: This descriptive study was carried out on 390 Isfahani parents with 6?14-year-old children. Data was collected through a questionnaire and interviews. The questionnaire consisted of questions on demographic data, the general oral health status in children, the general healthcare behavior, barriers to access to dental services, availability of and access to such services and parental satisfaction with dental services in their children. Data were analyzed with descriptive statistics and linear regression analysis using SPSS software (?=0.05). Results: Based on this study, 78% of children had access to dental care and 22.1% faced barriers, with the main barriers consisting of the high cost of dental treatments, insufficient dental insurance coverage and dental fears in children. The average satisfaction rate with dental services was 48%. Dental visits in 22% were due to regular dental checkups and 68% for dental and gingival problems. Conclusion: According to this study, the rate of access to dental care in the samples was over 70%. The main barriers to access to dental services based on parents’ reports were the cost of dental care, insufficient dental insurance coverage and dental fear. Half of the parents were satisfied with dental services provided for their children. Key words: Orodental care, Patient satisfaction, Access to health services.
