The comparison between Demineralized Bone Matrix and Absorbable Gelatin Sponge on preservation of the extraction socket bone level

سابقه و هدف: كاهش ابعاد ريج آلويول پس از خارج كردن دندان را مي توان با روش حفظ ساكت از طريق استفاده از مواد پيوندي به حداقل رساند. هدف از اين مطالعه مقايسه تاثير دو ماده Demineralized Bone Matrix وAbsorbable Gelatin Sponge بر حفظ ارتفاع استخوان ساكت دنداني پس از خارج كردن دندان مي باشد. مواد و روش‌ها: اين مطالعه با طراحي كارآزمايي باليني متقاطع دو سوكور بر روي 20 نمونه (10 بيمار) انجام شد. كليه بيماراني كه انديكاسيون خارج كردن مشابه در يك فك را داشتند و موافقت كتبي و آگاهانه خود را براي تحقيق اعلام نمودند مورد مطامعه قرارگرفتند. ابتدا پس از خارج كردن دندان هاي مشابه در يك فك، به طور تصادفي در يك ساكت دنداني ماده Demineralized Bone Matrix و در ساكت ديگر ماده Absorbable Gelatin Sponge قرار داده شد. سپس ميزان ارتفاع ريج آلويولار با راديوگرافي پري اپيكال موازي و ساخت استنت، به منظور ثبت شرايط تصويربرداري و تكرارپذيري آن، تعيين گرديد. پس از 4ماه به كمك استنت تهيه شده، مجددا راديوگرافي پري اپيكال موازي با همان روش قبل تهيه و ميزان ارتفاع استخوان ريج آلويول اندازه گيري گرديد. سپس ميزان تغييرات داخل گروهي هر ماده با آزمون paired t-test و تغييرات بين گروهي مواد با آزمون t-test مورد قضاوت آماري قرار گرفت. يافته‌ها: در هر دو گروه كاهش در ارتفاع ريج آلويول مشاهده گرديد؛ به نحوي كه ميزان آن در گروه Absorbable Gelatin Sponge برابر با (07/2 ± 03/3) ميليمتر و در گروه Demineralized Bone Matrix برابر با (47/0 ± 76/0) ميليمتر و اختلاف بين آن ها از نظر آماري معني دار بود. (01/0 > P) نتيجه‌گيري: ميزان تغييرات ارتفاع در گروه Absorbable Gelatin Sponge بيش از 3 برابر گروه Demineralized Bone Matrix بود كه نشانه عملكرد بهتر Demineralized Bone Matrix نسبت به Absorbable Gelatin Sponge در حفظ ارتفاع استخوان ريج آلويول پس از خارج كردن دندان مي باشد. كليد واژه ها: آلوگرافت، ريج آلويول، تحليل استخوان آلويول، تقويت ريج آلويول، خارج كردن دندان

ABSTRACT Background and Aim: Alveolar ridge resorption following tooth extraction could be minimized via socket preservation techniques by using grafting materials. The aim of this study was to compare the effect of Demineralized Bone Matrix and Absorbable Gelatin Sponge on preservation of the extraction socket bone level. Materials and Methods This double blind clinical trial study was accomplished on 20 cases (10 patients). The patients with the indication of two similar teeth extraction in a jaw who declared their written and conscious agreement for the research were included in this study. First, after similar teeth extraction in a jaw, Demineralized Bone Matrix material was embedded randomly into one extraction socket and Absorbable Gelatin Sponge on the other one. Then, the height of alveolar socket was measured by means of taking parallel periapical radiography and making a guiding stent. Four mounts later, another parallel periapical radiography was taken in the same manner as the previous one by using the fabricated guiding stent as guidance. Then the height of alveolar socket was measured. Differences inside and between the groups were analyzed statistically by using the “paired t-test” and “t-test” analyses respectively. Results: In both groups, reduction on the amount of alveolar ridge height observed. Bone level reduction in the Absorbable Gelatin Sponge and Demineralized Bone Matrix group was 3.03 ± 2.07 mm and 0.76 ± 0.47 mm, respectively, with statistically significant difference between them (P < 0.01). Conclusion: Bone level changes in Absorbable Gelatin Sponge group was 3 times more than Demineralized Bone Matrix group, which means that Demineralized Bone Matrix is more capable of preserving the bone height of the extraction socket than the Absorbable Gelatin Sponge.
