تشخيص بيماري‌هاي پوستي از راه دور: مقايسه‌ي صحت و پايايي تشخيص با معاينات حضوري

Teledermatology: A comparison of reliability and accuracy of diagnoses with those in the face to face visits

بیش از یك دهه است كه تشخیص بیماری‌های پوستی از راه دور به‌عنوان جایگزینی بالقوه برای معاینات حضوری مورد توجه قرار گرفته است. به‌منظور ارزیابی كیفیت این خدمات، پایایی و صحت تشخیص‌ها با روش معاینه‌ی حضوری مقایسه می‌شوند. در این مقاله نتایج مطالعات انجام‌شده در خصوص مقایسه‌ی پایایی و صحت تشخیص‌های ارایه‌شده در دو روش معاینه حضوری و از راه دور مرور می‌گردد. از منابع مختلف نظیر پایگاه‌های اینترنتی و مجلات الكترونیكی برای انتخاب مقالات منتشرشده در سال‌های 1997 تا 2013 در حوزه‌ی تشخیص بیماری‌های پوستی از راه دور با تمركز بر مقایسه‌ی پایایی و صحت تشخیص‌ها در روش از راه دور و معاینه‌ی حضوری استفاده شد. پایایی و صحت تشخیص از راه دور، در مقایسه با معاینات حضوری بیش از 60% بود. هم‌چنین افزایش پایایی و صحت به عواملی نظیر كیفیت تصاویر، اطلاعات بالینی، تشخیص‌های افتراقی، مشورت با سایر همكاران و كسب نظرات ثانویه آن‌ها بستگی داشت. به‌نظر می‌رسد ارایه‌ی اطلاعات بالینی كافی در خصوص بیماران به ارتقای پایایی و صحت تشخیص بیماری‌های پوستی از راه دور كمك خواهد كرد. در این صورت می‌توان از این روش جهت ارایه‌ی مراقبت‌های سلامت در مناطق دورافتاده و جمعیت‌های محروم استفاده نمود.

It is more than a decade that teledermatology has been considered as a potential substitute for the face to face visits. To evaluate its quality, the reliability and accuracy of teledermatology diagnoses are compared to those which are made in face to face visits. In this article, the results of the studies which have compared the reliability and accuracy of diagnoses had been made using teledermatology to the face to face visits were reviewed. Different resources, such as electronic databases and e-journals were used to choose relevant articles related to teledermatology published between 1997 and 2013, and their main focus was on the reliability and accuracy of diagnoses in teledermatology compared to those in the face to face visits. The reliability and accuracy of diagnoses in teledermatology compared to those in the face to face visits was more than 60%. Moreover, improving the reliability and accuracy depended on different factors, such as image quality, clinical information, differential diagnoses, consultation with other colleagues and gaining their secondary opinions. It seems that providing adequate clinical information about patients can help to improve the reliability and accuracy of diagnoses in teledermatology. In this case, teledermatology can be used to provide healthcare services for the distant areas and the poor.
