چالش‌هاي تعرفه گذاري نادرست خدمات پزشكي: مديران سلامت چه مي‌گويند؟؛ يك مطالعه كيفي

Health Managers\ʹ Perspective On The Challenges And Injuries Caused By Incorrect Medical Service Tariff, A Qualitative Study

زمینه و هدف: تعرفه‌گذاری بر عدالت، كارایی، كیفیت و پاسخگویی در ارایه‏ خدمات سلامت مؤثر بوده و با آن می‌توان بر میزان دسترسی و استفاده از خدمات تأثیر گذاشت. در ایران چند سالی است كه بحث تعرفه‌گذاری خدمات پزشكی به دلیل افزایشی بودن در هرسال و نداشتن مبنای پایه مشخص و قابل دفاع، باعث ایجاد چالش‌های مختلفی در نظام سلامت گردیده است. هدف از این پژوهش شناسایی چالش‌ها و آسیب های ناشی از تعرفه گذاری نادرست خدمات پزشكی بود. روش بررسی: این پژوهش یك مطالعه‏ی كیفی با رویكرد تحلیل محتوا است كه به صورت مقطعی در سال 1393-1392 انجام شده است. نمونه‌گیری به روش هدفمند انجام شده و تا اشباع داده‌ها ادامه یافت، جامعه پژوهش مدیران سلامت شهرهای تهران، اصفهان و تبریز بودند كه با 12 نفر از آنان در راستای هدف پژوهش به مصاحبه عمیق پرداخته شد. روش تحلیل داده‌ها در این پژوهش بر مبنای روش تحلیل موضوعی است. معیارهایی مانند «موثق بودن اطلاعات»، «قابل اعتماد بودن»، «قابل اطمینان بودن» و «قابل تثبیت بودن» داده ها در نظر گرفته شد. در مورد روایی؛ مصاحبه كننده تحت آموزش های لازم قرار گرفت و اطمینان از درست بودن انجام مصاحبه‌ها بالا رفت و در مورد پایایی مطالعه، مصاحبه‌ها مجدداً به مصاحبه شوندگان ارجاع داده شد تا نظر آنان درمورد صحت مصاحبه های پیاده شده به دست آید. یافته‌ها: مطابق یافته‌های پژوهش، چالش‌هایی كه ناكارآمدی تعرفه‌های خدمات پزشكی در شرایط فعلی باعث شده‌اند به سه دسته‏ی كلی شامل چالش‌های؛ نارضایتی ذی نفعان، اقتصادی و رفتاری در ارتباط با 4 حوزه وزارت بهداشت، پزشكان، بیماران و سازمان‌های بیمه‌گر می‌باشند. به عبارتی نارضایتی و چالش‌های اقتصادی و رفتاری در تمامی‌بخش های مربوطه مشاهده می‌شود.. نتیجه‌گیری: چالشهای حاصل از تعرفه گذاری نادرست و تأثیرات سوء آن بر تمامی‌ذی نفعان نظام سلامت، به عنوان هشداری برای متولیان و سیاستگذاران این نظام می‌باشد تا درصدد علت یابی و تلاش برای رفع این چالش‌ها و مشكلات برآیند. نارضایتی ذی‌نفعان و به ویژه ارایه كنندگان خدمات سلامت، به عنوان اولین عامل می‌باشد كه دیگر چالش‌ا شامل؛ چالش‌های اقتصادی و رفتاری را نیز در پی داشته است.

Background & aim: Tariff is effective on equity, efficiency, quality and accountability in the provision of services and it can affect the availability and use of services. In our country, for several years, the tariff of health care is due to an increasing base and has not clear and defensible base. This matter creates different challenges in our health system. This study aimed to identify challenges and damages resulting from incorrect medical services. Method: This study was a qualitative study .Sampling was Purposeful and continued until data saturation was obtained. Research Society were 12 people of Health Managers of cities including Tehran, Isfahan and Tabriz, that in the purpose of the study, these people were investigated with deep interview. Analysis of the data in the present study was based on a thematic analysis. Criteria such as "reliable information", "trustworthy", "reliability" and "ability tocon solidation" of data were considered. Results: according to the findings, inefficiency of tariffs for medical services have caused challenges in the current situation that this damages were divided to three general categories and challenges, dissatisfaction, economic, and behavioral of Ministry of Health, doctors, patients and insurers. Conclusion: Dissatisfaction with stakeholders, especially health providers, the first factor is that a real so following other challenges including; economic and behavioral challenges. Challenges of incorrect tariffs and their Bad effects on the health system stakeholders are a warning to administrators and policy makers of this system, finding causes and attempting to fix the challenges. Although incorrect medical tariff is introduced as one of the challenges causes, but it is necessary in addition to that are note other structural and Rooty problems in the health system, as noted tariff reforms to fully be able to solve the created challenges.
