ميزان رعايت استانداردهاي تشخيص‌نويسي در پرونده‌هاي پزشكي بيماران بستري در بيمارستان‌هاي تابعه دانشگاه علوم پزشكي قم

The Rate of Observance of the Standards of Diagnosis Recording in Medical Records of Hospitalized Patients in Hospitals Affiliated To Qom University of Medical Sciences, Iran

زمینه و هدف: مراقبت بهداشتی با كیفیت، بستگی به اطلاعات كامل و جامع پرونده پزشكی دارد. اما مراقبت با كیفیت بدون وجود اطلاعات صحیح، بسیار دشوار است. با توجه به اهمیت رعایت اصول تشخیص‌نویسی در پرونده‌های بالینی و استفاده‌های آموزشی، درمانی، تحقیقاتی، قانونی و آماری از آنها و عدم اطلاع از وضعیت موجود، این پژوهش به‌منظور تعیین میزان رعایت اصول تشخیص‌نویسی در پرونده‌های بالینی بیمارستان‌های تابعه دانشگاه علوم پزشكی قم انجام گرفت. روش بررسی: این پژوهش به روش توصیفی - تحلیلی در سال 1393 بر روی پرونده‌های پزشكی بیماران بیمارستان‌های تابعه دانشگاه علوم پزشكی قم (مشتمل بر 6 مركز) انجام شد. نمونه آماری، 398 عدد برآورد گردید. جهت گردآوری داده‌ها از چك‌لیست استفاده شد. داده‌ها به كمك آزمون دقیق فیشر و تی‌مستقل تجزیه و تحلیل شدند. یافته‌ها: در این مطالعه با بررسی میزان رعایت اصول تشخیص‌نویسی در پرونده‌های پزشكی، بیشترین موردی‌كه رعایت شده بود میزان رعایت اصول تشخیص‌نویسی با 58% بود. پس از آن، اصول ثبت اقدامات پزشكی با 50% و علل خارجی حوادث با 5/5% به ترتیب در مكان‌های دوم و سوم قرار داشتند. نتیجه‌گیری: با توجه به نتایج این مطالعه، توجه و اهتمام بیشتر مسئولین، پزشكان و كارشناسان مدارك پزشكی به این امر در بیمارستان‌های تابعه دانشگاه علوم پزشكی قم و برگزاری كلاس‌های آموزشی برای پزشكان، كادر پذیرش و پرستاری ضروری به‌نظر می‌رسد

Backgrounds and Objectives: High-quality health care depends on comprehensive information of medical record, and high-quality care is very difficult without accurate information. Given the importance of the observance of principles of diagnosis recording in clinical records and their educational, therapeutic, research, legal, and statistical application and given lack of knowledge about the existing situation, this research was conducted to determine the rate of observance of principles of diagnosis recording in the clinical records in the hospitals affiliated to Qom University of Medical Sciences. Methods: This research was conducted as a descriptive-analytical study on medical records of the patients of hospitals affiliated to Qom University of Medical Sciences (including 6 centers) in 2014. The number of statistical sample was estimated to be 398. Data were gathered using a checklist. Data were analyzed by Fisherʹs exact test and independent t-test. Results: In this study, assessment of the observance rate of principles of diagnosis recording in medical records, showed that the highest observance was for the observance rate of diagnosis recording principles with 58%. Then, recording principles of medical measures with 50%, and external causes of events with 5.5%, were respectively in the second and third levels. Conclusion: According to the findings of this study, it seems that more attention and effort of authorities, physicians, and experts of medical records to this issue in the hospitals affiliated to Qom University of Medical Sciences and holding educational classes for physicians, and admission staff and nurses, are necessary.
