ارزيابي دانش و مقايسه عملكرد پرستاران بخش ويژه در تغذيه لوله‌اي با استانداردهاي موجود در مراكز آموزشي درماني دانشگاه علوم پزشكي قم

Knowledge Assessment and Comparing the Performance of Intensive Care Unit Nurses in Regard to Tube Feeding with Existing Standards in Educational and Treatment Centers of Qom University of Medical Sciences, Iran

زمینه و هدف: تغذیه لوله‌ای یكی از روش‌های تغذیه حمایتی در بیماران بستری در بخش ویژه می‌باشد كه باوجود مزایای متعدد، عوارضی نیز برای بیمار به همراه دارد. دانش و عملكرد پرستاران نقش مهمی‌ در جلوگیری و ایجاد عوارض ناشی از تغذیه لوله‌ای دارد. مطالعه حاضر، به‌منظور تعیین دانش و مقایسه عملكرد پرستاران بخش ویژه در تغذیه لوله‌ای با استانداردهای موجود در مراكز آموزشی درمانی دانشگاه علوم پزشكی قم صورت گرفت. روش بررسی: در این مطالعه توصیفی، 68 نفر از پرستاران شاغل واجد شرایط در بخش‌های ویژه بیمارستان‌های دانشگاه علوم پزشكی قم (سال 1393)، با نمونه‌گیری دو مرحله‌ای وارد مطالعه شدند. جمع‌آوری اطلاعات با پرسشنامه‌ پژوهشگرساخته و چك‌لیست استاندارد‌ انجام شد. داده‌ها با استفاده از آزمون‌های آماری تی مستقل، آنالیز واریانس و تی‌تك نمونه‌ای، تجزیه و تحلیل شدند. سطح معنی‌داری، 05/0 در نظر گرفته شد. یافته‌ها: میانگین نمره دانش و عملكرد پرستاران ویژه در تغذیه لوله‌ای، متوسط بود. بین نمره عملكرد پرستاران مرد و زن، اختلاف آماری معنی‌داری وجود داشت (001/0p<). بین نمره عملكرد تغذیه لوله‌ای پرستاران كمتر از 30 سال و بالاتر از 30 سال، اختلاف آماری معنی‌داری دیده شد. مقایسه نمره عملكرد با استاندارد نشان داد میانگین نمره 2/15±56، اختلاف آماری معنی‌داری با نمره استاندارد در سطح كمتر از 001/0 دارد. نتیجه‌گیری: نتایج این مطالعه نشان داد میانگین نمره عملكرد پرستاران در تغذیه لوله‌ای در حد متوسط و به‌طور معنی‌دار كمتر از استاندارد بوده است. لذا اقدامات آموزشی و آموزش‌های  ضمن خدمت برای كادر پرستاری ضروری به‌نظر می‌رسد.

Background and Objectives: Tube feeding is one of the supportive nutritional methods in intensive care units (ICU), which is associated with some complications, despite numerous advantages for patients. Nurses’ knowledge and performance play an important role in preventing and causing tube feeding’s complications. The present study was performed with the intention of determining knowledge and comparing the performance of ICU nurses in regard to tube feeding with existing standards in educational and treatment centers of Qom University of Medical Sciences. Methods: In this descriptive study, 68 eligible nurses working in ICUs of hospitals of Qom University of medical sciences (2014), were entered into the study using two-stage sampling. Data collection was carried out using a researcher-made questionnaire and and a standard checklist was used for. Data were analyzed by statistical tests of independent t, one sample t-, and analysis of variance. The significance level was considered 0.05. Results: The mean score of knowledge and performance in ICU nurses was average. There was a significant difference between the performance scores of male and female nurses (p<0.001). A significant statistical difference was observed in the performance scores of tube feeding between nurses below and above 30 years of age. The comparison of performance score with the standard showed that the mean score of performance (56±15.2) had a significant difference from the standard score at level less than 0.001. Conclusion: The findings of this study indicated that the mean score of nurses’ performance for tube feeding was average and significantly lower than the standard. Therefore, training measures and in-service training seem to be necessary for nursing staffs.
