بررسي فرايند بستري و ترخيص بيمار با رويكرد كايزن و ارزيابي تصميم بر اساس معيار هاي چندگانه در يك بيمارستان

Assessing the Patientsʹ Hospitalization and Discharge Processes Based on Kaizen approach and Multiple-Criteria Decision Making (MCDM) in a Hospital

زمینه و هدف:افزایش تقاضا برای خدمات سلامتی و نیز افزایش هزینه های مرتبط، موجب تلاش بیمارستان ها برای ارتقاء و سازماندهی فرآیندها شده است.هدف از انجام این پژوهش تعیین فرآیند بستری و ترخیص بیمار بر اساس رویكرد كایزن و ارزیابی تصمیم گیری بر اساس معیار های چندگانه (MCDM) در بخش داخلی یك بیمارستان بود. مواد و روش ها:دراین مطالعه توصیفی–تحلیلی،ده نفر از مدیران پرسشنامه های مربوط به كیفیت فرآیندهای بستری و ترخیص را تكمیل نموده و به سه سوال باز در مورد روش های پیشنهادی بر اساس اصول كایزن پاسخ دادند. سپس میزان رضایتمندی 100 بیمار، با استفاده از پرسشنامه (پایایی93/0=α)،موردسنجش قرارگرفت. در مرحله سوم، پس از بررسی راه حل های پیشنهادی با روش فرآیند تحلیل سلسله مراتبی AHP، مدیران راه حل ها را با توجه به شاخص های هزینه، زمان و اثربخشی اولویت دهی نمودند. نتایج:نتایج نشان داد كه میانگین نمره فرآیند بستری از دیدگاه مدیران 52/88 و فرآیند ترخیص 33/90 (از 100) بود كه نشان دهنده میزان مطابقت بالا با رویكرد كایزن بود. همچنین میانگین رضایت بیماران از بستری 75/76 (از 100) واز ترخیص 6/78 بود كه نشاندهنده رضایت مناسبی است. نتایج روش MCDM و AHP نشان داد كه از دیدگاه مدیران شاخص اثربخشی به عنوان مهمترین و سپس زمان و هزینه  اولویت های بعدی برای اجرای راه حل های پیشنهادی هستند. نتیجه گیری:این مطالعه نشان داد فرآیندهای بستری و ترخیص در بخش داخلی بیمارستان مورد پژوهش، اگرچه از وضعیت نسبتا مطلوبی برخوردار بودند اما همچنان لازم است برای بهبود فرآیندهای بستری و ترخیص،بهترین راه حل ها از نظر اثربخشی، زمان و هزینه  اتخاذ و اجرا شوند تا رضایت كامل بیماران را به همراه داشته باشند.

Background: Enhancing the need for health care services and its related costs, lead hospitals to improve and organize their processes. The aim of current study was to assess the Patientsʹ Hospitalization and Discharge Processes Based on Kaizen approach and Multiple-Criteria Decision Making (MCDM) in an internal ward of one Hospital. Materials and Methods: In this descriptive analytical study, ten managers completed the hospitalization and discharge questionnaires and answered three open questions according to Kaizen principles. Then, satisfaction score was measured for 100 patients using a reliable questionnaire (α=0.93). At third phase, after evaluating the proposed solutions using Analytic Hierarchy Process (AHP), the managers prioritized them based on cost, time and effectiveness considering indicators. Results: Study findings revealed that mean scores of activity in hospitalization and discharge processes were 88.52 and 90.33 (out of 100) respectively, which showed high conformity with Kaizen approach. Also, mean scores of hospitalization and discharge satisfaction which indicated desirable satisfaction were 76.75 and 78.6 (out of 100), respectively. Based on managersʹ view points, results of MCDM and AHP methods indicated that effectiveness was the most important indicator and time as well as cost gained next priorities for solutions implementation. Conclusion: This study showed that although, hospitalization and discharge processes were relatively desirable in the internal ward in mentioned hospitals, it is still a necessary to seek the best solutions in terms of effectiveness, time and cost for patientsʹ satisfaction.
