Safety assessment of anti-dermatitis creams by measuring the biophysical parameters of skin: Phase I clinical trial with control group

زمینه و هدف: درماتیت یا اگزما نوعی اختلال پوستی خارش‌دار است كه معمولاً به‌علت واكنش التهابی پوست در پاسخ به تركیبی از عوامل ژنتیكی و محیطی ایجاد می­شود. این مطالعه با هدف ارزیابی بی‌خطری كرم‌های ضددرماتیت و بررسی عوارض جانبی احتمالی روی داوطلبین انسانی با اندازه‌گیری شاخص‌های بیوفیزیكی پوست به‌عنوان روشی غیرتهاجمی صورت پذیرفت. روش اجرا: در كارآزمایی بالینی فاز I حاضر، دو گروه 15 نفره‌ی داوطلب بزرگسال و 15 داوطلب كودك سالم جهت بررسی بی‌خطری سه كرم ضددرماتیت آتوپیك و تماسی انتخاب شدند. اندازه‌گیری‌ها به كمك دستگاهMPA 580 ® Cutometerصورت گرفت و تغییرات رخ‌داده در شاخص‌های میزان تبخیر آب از اپی‌درم، رطوبت لایه‌ی شاخی، محتوای ملانین، محتوای هموگلوبین (اریتم) و pH پوست ثبت گردید. این تغییرات در هر یك از متغیرها در سمت مصرف كرم و سمت شاهد، با آزمون­های آماری مقایسه شدند و 05/0P< معنادار تلقی شد. یافته‌ها: در مقایسه‌ی انجام‌گرفته هیچ‌گونه تفاوت آماری معناداری در شاخص‌های میزان تبخیر آب از اپی‌درم، محتوای ملانین، محتوای هموگلوبین و pH مشاهده نشد. اختلاف بین تغییر رطوبت لایه‌ی شاخی در سمت مصرف كرم­های Dermalex Repair Eczema، Dermalex Repair Contact Eczema وDermalex Repair Eczema for Babies & Children با سمت شاهد معنادار تلقی گردید و P حاصل به‌ترتیب برابر 002/0، 001/0 و 013/0 بود كه به‌دلیل وجود تركیبات پوشاننده یا جذب‌كننده‌ی رطوبت در كرم‌ها بود. نتیجه‌گیری: با توجه به آن كه پس از استفاده،تغییرات نامطلوبی در شاخص‌های بیوفیزیكی پوست ایجاد نشد می‌توان هر سه كرم را بی ضرر در نظر گرفت. نتایج خودارزیابی داوطلبین نیز بر ایجاد نشدن عوارض جانبی دلالت داشت.

Background and Aim: Dermatitis or eczema is an itchy skin disorder which is usually caused by the skin inflammatory reaction in response to combination of genetic and environmental factors. This study was aimed to assess the safety of anti-dermatitis creams and possible side effects on human volunteers through measuring the skin biophysical parameters with non-invasive methods. Methods: Two groups of 15 adults and 15 children as healthy volunteers participated for safety assessment of three anti atopic and contact dermatitis creams. All measurements were done using Cutometer® MPA 580 and the changes in the rate of transepidermal water loss (TEWL), stratum corneum hydration, content of melanin and hemoglobin (erythema) and skin pH were recorded. The obtained data were analyzed using SPSS software version 20 and P<0.05 were considered as significant. Results: In this trial, no statistically significant changes in skin biophysical parameters including TEWL, melanin, erythema and pH was observed. The stratum corneum hydration on the applicatioins areas of Dermalex Repair Eczema, Dermalex Repair Contact Eczema, Dermalex Repair Eczema for Babies & Children compared to the control sides were significantly different (P=0.002, 0.001 and 0.013). This significant difference was due to emollient or humectant compounds in these creams. Conclusion: Because of no adverse changes in the skin biophysical parameters, the studied anti-dermatitis creams could be conisdered as safe products. In addition, due to self-assessment results of adverse effects by the participants, no significant adverse effect was seen.
