پيوند زندگي با ماستكتومي: تبيين تجارب سازگاري با ماستكتومي در زنان جوان تحت شيمي‌درماني – گراندد تئوري

TRANSPLANTATION LIVING WITH MASTECTOMY: EXPLAINING THE EXPERIENCES ADAPTATION WITH MASTECTOMY OF YOUNG WOMEN UNDERGOING CHEMOTHERAPY

پيش زمينه و هدف: ماستكتومي رايج ترين درمان براي سرطان پستان در ايران مي باشد. لذا با افزايش تعداد اين بيماران اهميت بررسي روند سازگاري آن ها آشكار مي گردد. هدف از اين مطالعه تبيين روند سازگاري با ماستكتومي در زنان جوان تحت شيمي درماني مي باشد. مواد و روش كار: اين مطالعه با رويكرد گراندد تئوري مي باشد كه مشاركت كنندگان به صورت نمونه گيري هدفمند و نظري انتخاب شدند. داده ها از طريق مصاحبه نيمه ساختاريافته جمع آوري و تا زمان اشباع داده ها ادامه يافت. داده ها به طور همزمان با جمع آوري اطلاعات با استفاده از روش اشتراوس و كوربين تحليل گرديد. اعتبار داده ها بر اساس معيارهاي پيشنهادي ليكنولن و گوبا تأييد شد. يافته ها: تجارب روند سازگاري 23 مشاركت كننده در 5 درون مايه شامل مواجهه با بحران (بستر)، فراز و نشيب ها (شرايط علي)، از دست دادن هويت (پديده)، حمايت همه جانبه (عوامل مداخله گر)، تلاش براي بازسازي زندگي (راهبرد) و پيوند زندگي با ماستكتومي به عنوان پيامد مي باشد كه داراي زير طبقات بود. بحث و نتيجه گيري: تطابق مشاركت كنندگان با ماستكتومي از يك فرايند پيچيده تبعيت مي كند. بيماران به دليل از دست دادن ناگهاني سلامتي و تغيير در تصور از جسم خود، تنش هاي و محدوديت هاي زيادي را تجربه مي كنند. لذا آگاهي كادر درمان از اين فرايند مي تواند در جلسات مشاوره و آموزش به بيمار جهت تسهيل پذيرش و اتخاذ رويكردهاي مثبت مقابله اي و بازگشت به زندگي آنان مؤثر باشد.

Therefore, with increasing numbers of patients, evaluating their compatibility becomes apparent. The purpose of this study was to explain process of the compatibility with mastectomy in young women undergoing chemotherapy. Materials Methods: This study is a grounded theory approach that participants were chosen purposefully based on theoretical sling. The data were collected through semistructured interviews and the process were continued until data saturation. The data simultaneously were collected and analyzed by using Strauss and Corbin approach. Validity of the data were confirmed based on Lincoln and Goba criteria. Results: The findings showed that the process of compatiblity in 23 young women participating mastectomy undergoing chemotherapy can be catogorized in 5 themes including crisis (context) ups and downs (condition), and loss of identity (phenomenon), vast support (confounders), trying to rebuild lives (strategy) and the Transplantation alongside living with mastectomy, which inturn had subcategories. Conclusion : The study revealed that process of compliance of young women with mastectomy obeys a complex process . By paying attention to t hese patients who have the fear of sudden health loss, and change in the perception of the body image, one can understand that they experienced severe stress and limitations. Therefore, the medical team can inform this process to the patients by counseling and educating them to facilitate its acceptance and adoption.
