تجزيه و تحليل مفهوم هندلينگ كاردرماني در كودكان فلج مغزي: يك مطالعه هيبريد

Concept Analysis of Occupational Therapy Handling in the Children with Cerebral Palsy: A Hybrid Model

  هدف: هدف از اين مطالعه تجزيه و تحليل مفهوم هندلينگ كاردرمانی در كودكان فلج مغزی ، براساس تجربه كاردرمانگران بالينی و مدرسان كاردرمانی در جامعه ايرانی بود.   روش بررسی: اين مطالعه از نوع كيفی است كه تجزيه و تحليل مفهوم هندلينگ در كودكان فلج مغزی به روش هيبريد، در سه مرحله انجام شد. در مرحله نظری، مروری بر مقالات مرتبط موجود در پايگا ه ‌های اطلاعاتی معتبر تا سال 2014 صورت گرفت و مقالات ارزيابی و تحليل شد . در مرحله كار در عرصه، نمونه گيری بصورت هدفمند و از طريق مصاحبه عميق و نيمه‌ساختارمند با 6 كاردرمانگر بالينی، ميزگرد بحث و گفتگوی تخصصی با حضور 11 كاردرمانگر و دو مشاهده در كلينيك صورت گرفت. سپس نتايج دست‌نويس شد و تحليل محتوا صورت گرفت . در مرحله نهايی، تحليل كلی از دو مرحله قبل به انجام رسيد .   يافته‌ها: پنج درونمايه اصلی برای مفهوم هندلينگ استخراج شد كه عبارتند از: « مراقبت از كودك فلج مغزي»، «مديريت درمان»، «درمان­های دستي»، « آموزش مهارت‌های روزمره زندگی » و «حمل و جابه‌جايي». هم­چنين درونمايه‌های مربوط به ويژگی ‌ های هندلينگ براساس نظرات مشاركت‌كنندگان؛ كنترل، ايمنی، حركت و وضعيت‌دهی شناسايی شدند .   نتيجه‌گيری: به‌نظر می ‌رسد نتايج اين پژوهش می‌تواند تا حدودی به واضح‌ تر شدن مفهوم هندلينگ كاردرمانی در كودكان فلج مغزی كمك نمای د . همچنين با شناسايی فرايند عوامل تسهيل كننده، بازدارنده و مفهوم هندلينگ، مدرسين و كاردرمانگران بالينی خواهند توانست با تكيه بر دانش مبتنی بر يافته‌های علمی، فعاليت‌های درمانی و آموزشی خود را به‌گونه‌ای طراحی و اجرا كنند كه شرايط لازم برای آموزش، يادگيری و اجرای صحيح هندلينگ در كاردرمانی فراهم آيد .

  Objective: This study aimed to analyze the concept of occupational therapy handling in the children with cerebral palsy from the perspective of occupational therapy instructors and clinicians in Iran.   Materials and methods : In this qualitative study, using hybrid model to clarify the concept of handling through three phases. For the theoretical phase , attributes of handling were recognized through a review of the literature (until February 2014), and six in-depth semi - structured interviews, two observations and one panel of expert s were conducted for the fieldwork to develop attributes from the data and to verify those identified from the literature review. In the third phase attributes and final analysis of handling were extracted from the first and second phase.   Results: The results were classified in five main categories that were identified as : (1) care of child, (2) management of treatment, (3) manual technique s , (4) education of activities of daily living (ADL), and (5) lifting and carrying. Core attributes of handling include "control, safety, transfer and positioning ".   Conclusion: It seems that the result s of this study may help in clarifying the concept of handling in children with CP. In addition, by identifying the process, barriers and facilitative factors, and the concept of handling, occupational therapy instructors and therapists will be able to design and run their educational activities based on scientific findings which can provide them with the necessary conditions for education, learning and proper execution of handling in occupational therapy.
