تأثير روش حساسيت زدايي با حركات چشم و پردازش مجدد بر اضطراب نوجوانان مبتلا به تالاسمي

Effectiveness of Eye Movement Desensitization and Reprocessing on Anxiety in Children with Thalassemia

مقدمه: بيماري تالاسمي از بيماري هاي شايع ژنتيكي در جهان و به ويژه در كشور ما مي باشد. ﺑﺴﻴﺎري از دﻻﻳﻞ مانند ﻣﺰﻣﻦ ﺑﻮدن ﺑﻴﻤﺎري، ﻫﺰﻳﻨﻪ درﻣﺎﻧﻲ، ﺣﺎﻻت ﺑﻴﻤﺎري اﻧﺘﻈﺎر مرگ زودرس ﺑﺎﻋﺚ اضطراب در ﺑﻴﻤﺎران ﺗﺎﻻﺳﻤﻲ ﻣﻲﮔﺮدد. پژوهش حاضر ﺑﺎ ﻫﺪف ﺗﻌﻴﻴﻦ ﺗﺄﺛﻴﺮ ﻛﺎرآيي روش ﺣﺴﺎﺳﻴت زداﻳﻲ ﺑﺎ ﺣﺮﻛﺎت ﭼﺸﻢ و ﭘـﺮدازش ﻣﺠـﺪد ﺑـﺮ ﻣﻴـﺰان اﺿﻄﺮاب نوجوانان ﻣﺒﺘﻼ ﺑﻪ تالاسمي اﻧﺠﺎم شد. روش ها: اين مطالعه يك كارآزمايي باليني مي باشد. 60 نوجوان مبتلا به تالاسمي در محدوده سني 12-20 سال انتخاب و به طور تصادفي در دو گروه قرار گرفتند. از ﭘﺮﺳﺸﻨﺎمه اطلاعات جمعيت شناختي و پرسشنامه اضطراب بك جهت گردآوري داده استفاده گرديد. در گروه مداخله درﻣﺎن ﺣﺴﺎﺳﻴﺖ زداﻳﻲ ﺑﺎ ﺣﺮﻛﺎت ﭼﺸﻢ و ﭘﺮدازش ﻣﺠﺪد در سه ﺟﻠﺴﻪ ﺑﻪ ﺻﻮرت ﻳﻚ روز در ﻣﻴﺎن ﺑﻪ ﻣﺪت 4530 اﺟﺮا شد. در گروه شاهد هيچ اقدامي انجام نشد. تجزيه و تحليل داده ﻫـﺎ ﺑـﻪ وﺳـﻴﻠﻪ آمار توصيفي، آزمون هاي آماري تي مستقل و آزمون تي زوجي، آزمون ناپارامتري و آزمون كولموگروف اسميرنوف انجام شد. يافته ها: نتايج نشان داد كه ميانگين ميزان اضطراب نوجوانان مبتلا به تالاسمي قبل از اجراي روش درماني ﺣﺴﺎﺳﻴﺖ زداﻳﻲ ﺑﺎ ﺣﺮﻛﺎت ﭼﺸﻢ و ﭘﺮدازش ﻣﺠﺪد در گروه مداخله 5/62 ±34/73بوده كه پس از انجام روش درماني ﺣﺴﺎﺳﻴﺖ زداﻳﻲ ﺑﺎ ﺣﺮﻛﺎت ﭼﺸﻢ و ﭘﺮدازش ﻣﺠﺪد به 2/48± 19/90تغيير يافت. كه نسبت به قبل از مداخله كاهش چشمگيري داشته است و از نظر آماري معنادار بود (001/0>P). همچنين نتايج نشان مي دهد كه ﺣﺴﺎﺳﻴﺖ زداﻳﻲ ﺑﺎ ﺣﺮﻛﺎت ﭼﺸﻢ و ﭘﺮدازش ﻣﺠﺪد به صورت معناداري در كاهش اضطراب نوجوانان مبتلا به تالاسمي موثر است. نتيجه گيري: مطالعه حاضر نشان داد درمان حساسيت زدايي چشم و پردازش مجدد از طريق تأثير بر يكپارچه سازي حافظه مي تواند عمل نمايد و اين روش در كاهش اﺿﻄﺮاب نوجوانان ﻣﺒﺘﻼ ﺑﻪ تالاسمي موثر است.

Introduction: Thalassemia is one of the most common genetic diseases in the world and especially in our country. For many reasons, including chronic disease, health care costs, fear of early death and disease statues led to anxiety. This study aimed to determine the efficacy of eye movement desensitization and reprocessing method on decreasing the anxiety of thalassemic adolescent. Methods: This study was a clinical trial. 60 thalassemic adolescent between 2012 years old were randomly assigned to two groups. Demographic questionnaire and Beck Anxiety Inventory were used to collection of data. In three sessions, the eye movement desensitization and reprocessing method were used in intervention group every other day for 45 to 30 minutes in each session. In control group no action were taken. For analysis of the obtained data, descriptive statistics, ttest, paired ttest, nonparametric tests and KolmogorovSmirnov tests were used. Results: the results showed that the average level of anxiety of thalassemic adolescent before implementing of EMDR therapy in intervening group was 34.73±5.62 which after EMDR therapy changed to 19.90 ± 2.48. It shows an obvious reducing of anxiety compared to preintervention and the results were statistically significant (P0<001). Besides, the results showed that EMDR was effective on decreasing the symptoms of anxiety in thalassemic adolescent Conclusion: The current study showed that EMDR can be useful through having the influence on memory integration and it is an effective method for treat or reduce anxiety among thalassemic adolescent.
