بررسي تأثير حساسيت زدايي با حركات منظم چشم و پردازش مجدد بر استرس بيماران تحت درمان با همودياليز

Effect of Eye Movement Desensitization and Reprocessing on Stress in Patients Undergoing Hemodialysis

مقدمه: بيماران همودياليزي، سطوح بالايي از استرس را تجربه ميكنند. اين مطالعه به منظور تعيين تأثير حركات منظم چشم و پردازش مجدد بر ميزان استرس اين بيماران انجام شد. روش كار: اين مطالعه كار آزمايي باليني است كه بر روي 90 بيمار تحت همودياليز بيمارستان شهيد رجايي كرج در سال 95-1394 انجام شد. روش نمونه گيري به صورت نمونه گيري آسان بود. سپس بيماران بطور تصادفي دريكي از دوگروه آزمون (45 نفر) و كنترل (45 نفر) قرار گرفتند. روش حساسيت زدايي با حركات چشم و پردازش مجدد بر روي گروه آزمون به مدت 30 تا 45 دقيقه انجام گرديد. گروه كنترل فقط مراقبت معمول را دريافت ميكردند. ابزار مورد استفاده پرسشنامه استاندارد سنجش استرس همودياليز بالدري بود كه توسط مشاهده گر كه نسبت به تخصيص نمونه هاي گروه هاي مورد مطالعه بي اطلاع بود قبل و بعد از مداخله اندازه گيري و ثبت گرديد. تجزيه و تحليل داده ها به وسيله نرم افزار SPSS 21 و توسط آمار توصيفي و آزمونهاي آماري تي مستقل، تي زوجي و كاي اسكوئر تجزيه و تحليل شدند. يافته ها: ميانگين و انحراف معيار نمره استرس در گروه آزمون قبل از 66/16± 77/15EMDR و بعد از 22/45&± 7/38EMDR بوده است. تجزيه و تحليل داده ها بين ميانگين امتيازات استرس نمونههاي پژوهش قبل و بعد از مداخله در گروه آزمون تفاوت معني داري را نشان داد (0/001= P) درحالي كه اين تفاوت درگروه كنترل معني دار نبود. نتيجه گيري: يافته ها بيانگر آن است كه روش حساسيت زدايي با حركات چشم و پردازش مجدد از جمله روشهايي است كه در بيماران تحت همودياليز كاهش استرس را در رويه درمان به دنبال خواهد داشت. استفاده از اين روش درماني در بيماران تحت همودياليز منجر به ارتقا سلامت و كاهش استرس آنها خواهد گرديد.

Introduction: Patients on hemodialysis experience high levels of psychological stress. The aim of this study was to determine the effect of eye movement desensitization and reprocessing (EMDR) on the stress of patients undergoing hemodialysis. Methods: The present study was a randomized clinical trial that was carried out on ninety patients undergoing hemodialysis at Shahid Rajaei Hospital, Karaj during year 201516. The subjects were selected using the purposeful sampling method. Then, the patients were randomly allocated to two groups; control group (n = 45) and experimental group (n = 45). Desensitization method with eye movements and reprocessing was performed by the experimental group for around 30 to 45 minutes. Patients in the control group received routine care of the study setting. Data were collected using the Hemodialysis Stress Scale questionnaire (HSSBaldree) before and after the intervention by a researcher, who was blind to the group the patients were allocated to. Data were analyzed using descriptive statistics, MannWhitney U, Fishers Exact Test, Ttest, and Chisquare tests with the SPSS 21 software. Results: The mean of anxiety in the experimental group was 66.77 &±; 16.15 before the intervention and 38.22 &±; 7.45 after the intervention. The mean score of stress among the participants showed a significant difference pre and postintervention in the experimental group (P < 0.001). However, this difference was not significant in the control group. Conclusions: The findings indicate that the desensitization method with eye movements and reprocessing decreases patients stress during treatment procedures. The use of EMRD for patients undergoing hemodialysis will lead to improved health and reduces their stress.
