تجارب توانبخشي مبتني بر منزل در بيماران با ضايعات نخاعي تروماتيك: يك مطالعه كيفي

Exploring HomeBased Rehabilitation in Traumatic Spinal Cord Injury Patients: A Qualitative Study

مقدمه: مراقبت توانبخشي يك پديده بين رشته ايي و وابسته به زمينه فرهنگي اجتماعي است كه در بازگشت مجدد سلامتي در مصدومين با ضايعه نخاعي تروماتيك وآماده كردن بيمار براي برگشت مجدد به زندگي نرمال و اجتماع، نقش بسزايي دارد. نياز به كشف ابعاد مختلف توانبخشي در بيماران ضايعات نخاعي در منزل در مطالعات متعدد مورد تاكيد بوده و شناخت آن به ارائه خدمات مؤثرتر كمك مي نمايد. لذا اين مطالعه با هدف تبيين تجارب توانبخشي مبتني بر منزل بيماران با ضايعات نخاعي تروماتيك انجام شده است. روش كار: مطالعه حاضر يك مطالعه با رويكرد كيفي در سال 1393 در بيمارستان هاي شهر تهران انجام شد. مشاركت كنند گان شامل 27 نفر كه 12 نفر از مصدومين حوادث ترافيكي با تروماي ستون فقرات (ضايعه نخاعي) به علت حادثه ترافيكي،2 مراقب خانوادگي، 5 پرستار، 4 پزشك، 2 فيزيو تراپ، و 2 كاردرمان در بخشهاي بستري و سرپايي تروما و اعصاب، بودند. از روش نمونه گيري هدفمند استفاده گرديد. جمع آوري داده ها از طريق مصاحبه نيمه ساختار يافته فردي انجام گرفت. داده ها بر اساس رويكرد تجزيه و تحليل محتواي قردادي، مورد تجزيه و تحليل قرار گرفت. يافته ها: تحليل داده ها منجر به ظهور 4 طبقه اصلي شامل فرآيند توانبخشي مبتني بر منزل بيماران با ضايعات نخاعي تروماتيك شامل چهار طبقه اصلي شامل محدوديت ها، تسهيل كننده هاي بهبودي، جستجوي حمايت و بازگشت چالشي به جامعه استخراج گرديد. نتيجه گيري: حمايت در منزل از بيماران با ضايعات نخاعي با تشكيل تيمي جهت ارائه مراقبت حمايتي متشكل از همتايان با ضايعات نخاعي، پزشك، پرستار، مددكار اجتماعي، روانشناس و روحاني ضروري به نظر مي رسد.

Introduction: Rehabilitation is an interdisciplinary field of study and is dependent upon the culture of the place of study, and has a significant role in the patient’s wellbeing and their return to work and normal life. The sequence of a phenomenon rehabilitation care and sociocultural context in which returning veterans with spinal cord injury, traumatic health and prepare the patient to return to normal life and this community have an important role to explore various aspects of rehabilitation. Several studies have investigated spinal cord injuries that take place at home with the aim of providing more effective services. Therefore, this study aimed to explore patients with spinal cord injuries, for whom traumatic homebased rehabilitation was performed. Methods: A qualitative study was conducted during year 2014 in Tehrans hospitals. Participants included 12 victims of traffic accidents with injuries of the spine (spinal cord injury), two family caregivers, five nurses, four doctors, two physiotherapists, and two occupational therapists in inpatient wards and outpatient trauma centers that explained the process of care based rehabilitation at home for patients with spinal cord injury. Firstly, purposive sampling was performed and was continued by theoretical sampling on the basis of codes and classes. Data were collected through individual semistructured interviews. Data were analyzed by the conventional content analysis approach. Results: Data analyzes led to the emergence of four main categories including homebased rehabilitation process of patients with traumatic spinal cord injury, consisting of four main subcategories: facilitator of recovery, limitations, seek support and challenging return to society. Conclusions: Support of spinal cord injury patients at home with a team of doctors, nurses, social workers, psychologists and clergy is necessary.
