مطالعه كيفي دلايل و پيامدهاي بيماري رواني

A Qualitative Study of the Causes and Consequences of Psychopaths based on the experience of their Companion

با گذار از رويكرد مكانيكي و روان تني به رويكرد روان تني اجتماعي، بيماري رواني به منزله پديده اي چند وجهي و امري فرهنگي ـ اجتماعي دركانون توجه قرار گرفته است. اين پژوهش درصدد است، با اتخاذ رويكرد تفسيرگرايي اجتماعي، به كشف و بررسي دلالت هاي ذهني و معنايي تجربه و درك همراهان بيمار رواني بپردازد. داده هاي اين مطالعه كيفي ازطريق مصاحبه نيمه ساخت يافته گردآوري و از روش نظريه زمينه اي براي تحليل داده ها استفاده شده است. به كمك روش نمونه گيري نظري و هدفمند با 19 نفر از اعضاي خانواده بيماران رواني مصاحبه شد و در نهايت داده هاي گردآوري شده در قالب 18مقوله محوري و يك مقوله هسته كدگذاري و تحليل شد. برمبناي نتايج پژوهش، مصاحبه شوندگان بيماري رواني را حاصل زمينه هايي چون سابقه بيماري رواني، گسيختگي خانواده، تجارب دردناك، نابساماني اقتصادي، رفتارهاي نامتعارف و زوال شبكه هاي حمايتي مي دانند. همراهان در مواجهه با حضور بيمار رواني در خانواده به كنش هاي متفاوتي براساس پذيرش يا عدم پذيرش بيماري رواني دست مي زنند كه دامنه آن از كتمان و انكار تا ترحم و پذيرش و درمان بيماري در نوسان است. بازسازي معنايي تجربه و درك همراهان بر غيرعادي بودن بيماري رواني، دوري گزيني، شدت و چندوجهي بودن اختلال رواني دلالت دارد. براساس مطالعه حاضر، وجود بيمار رواني پيامدهايي چون داغ ننگ افتخاري، تمسخر، طرد اجتماعي خودخواسته، فشار مالي مضاعف و گسيختگي مضاعف خانواده را براي اطرافيان بيمار درپي داشته است.

Passing from mechanical and psychophysical approaches to social psychophysical approach, psychological diseases have been considered as a multi-aspect phenomenon and a cultural-social issue. This research aims to study and discover mental and conceptual implications of experience and perception of families having mental patients using a social interpretation approach. Data of this qualitative study were collected through semi-structured interviews, and for data analysis grounded theory method is used. Using theoretical and purposive sampling, 19 family members of mental patients were interviewed and finally collected data were analyzed in the framework of 18 pivotal categories and one core category. Based on study results, those interviewed considered mental patients the result of different conditions and backgrounds such as history of mental diseases, family disintegration, painful experiences, economic problems and abnormal behaviors and dissolution of support systems. Attendants in confrontation with presence of mental patients in the family resort to different reactions based on acceptance or non-acceptance of the mental patients, in a range from concealing, negating to accepting the disease and attempt for treatment. Conceptual reconstruction of experience and perception of patients’ families indicate the abnormality of mental illness, isolation and loneliness, as well as criticality of mental disease. Based on this study, presence of a mental patient has led to consequences such as stigma, fear of stigma, derision, self-desired social isolation, financial stress and family disintegration for patients’ relatives.
