بررسي ارتباط شايستگي فرهنگي و مسئوليت پذيري با نگرش پرستاران نسبت به رعايت حقوق بيماران: نقش ميانجي باورهاي اخلاقي

The Relationship of cultural competence and responsibility with Nurses Attitude toward Patientיs Right: The mediating Role of Ethical Beliefs

شایستگی فرهنگی، مسئولیت پذیری و باورهای اخلاقی از عوامل تاثیر گذار در ارتقای كیفیت خدمات پرستاری هستند و می توانند نقش اساسی در رعایت حقوق  بیماران داشته باشند. لذا این پژوهش با هدف ارائه مدل پیش‌بینی رعایت حقوق بیماران توسط پرستاران براساس شایستگی فرهنگی، مسئولیت پذیری و باورهای اخلاقی انجام شد. روش پژوهش مطالعه ، همبستگی بوده و نمونه آماری شامل 300 نفر از پرستاران شاغل در علوم پزشكی استان آذربایجان غربی در سال 1395 بودند كه به روش نمونه‌گیری خوشه‌ای چند مرحله‌ای انتخاب شدند. برای گردآوری داده‌ها از پرسش نامه شایستگی فرهنگی پرستاران Perng و Watson ، پرسش نامه مسئولیت پذیری Mergler و Shield، پرسش نامه محقق ساخته باورهای اخلاقی و مقیاس حقوق بیماران محمودی و همكاران استفاده شد. داده‌ها با استفاده از آزمون ضریب همبستگی پیرسون و بوت استراپ از طریق نرم افزارهای SPSS و AMOS نسخه 22  مورد تحلیل قرار گرفتند. یافته ها حاكی از برازش مدل بوده و اثر مستقیم متغیرهای شایستگی فرهنگی (11/0)، مسئولیت پذیری(57/0) و باورهای اخلاقی (24/0) بر روی نگرش پرستاران نسبت به رعایت حقوق بیماران معنادار به دست آمد. اثرهای غیر مستقیم شایستگی فرهنگی(03/0)  و مسئولیت پذیری(02/0) با میانجی گری باورهای اخلاقی معنادار بودند. در مجموع 65 درصد از واریانس نگرش پرستاران نسبت به رعایت حقوق بیماران از طریق مدل پژوهش تبیین شد. با توجه به اثر مستقیم و غیرمستقیم شایستگی فرهنگی، مسئولیت پذیری و باورهای اخلاقی بر میزان نگرش پرستاران نسبت به رعایت حقوق بیماران، به نظر می‌رسد با تقویت باورهای اخلاقی می توان ارتباط شایستگی فرهنگی و مسئولیت پذیری را با نگرش پرستاران نسبت به رعایت حقوق  بیماران افزایش داد.

The cultural competence, responsibility and ethical beliefs are influential factors in providing quality services by nurses and pay an important role in patientʹs right. The purpose of this research was to illustarte a predictive model of patientʹs right based on cultural competence, responsibility and ethical beliefs. The research method was correlational study conducted in 2016 among 300 nurses in west Azerbaijan in 2016, selected by multistage cluster sampling. For gathering the data, the Perng and Watson’s nurses’ cultural competence questionnaire, and the Mergler and Shield responsibility questionnaire as well as the Mahmoudi and et al questionnaire on ethical beliefs and Scale rights of patients were used. The data were analyzed by using Pearson correlational coefficient and bootstrap through SPSS software and amos-22. The results indicated that the model was fitted and has direct effect in cultural competence (0.11), responsibility (0.57) and ethical beliefs (0.24) on nurse’s attitude with the rights of patients were significant. The indirect effects of cultural competence (0.03) and responsibility (0/03) with mediating of forming the model of ethical beliefs were significant. In conclusion, %65 of the variance in nurses’ attitudes to the patientsʹ rights was determined by the variables of this research model. Due to the direct and indirect effects of cultural competence, responsibility and ethical beliefs on the attitudes of nurses towards patientsʹ rights, it is important to increase cultural competence relationship and responsibility with nurses’ attitudes towards   patient’s rights by focusing on ethical beliefs.
