بررسي ميزان تحقق استانداردهاي فدراسيون جهاني آموزش پزشكي (WFME) در دوره دستياري داخلي يكي از دانشگاه هاي علوم پزشكي تهران

Assessment of the Implementation of the Standards of the World Federation of Medical Education (WFME) during the Internship Period of One of the Tehran University of Medical Sciences

مقدمه: در سند چشم انداز بيست ساله و برنامه پنجم توسعه كه با هدف ارتقاي جايگاه جهاني ايران تدوين گرديده، بر بهداشت و درمان تاكيد مي شود. يكي از معتبرترين الگوهاي اعتبار بخشي بين المللي در آموزش پزشكي، الگوي اعتباربخشي فدراسيون جهاني آموزش پزشكي (WFME) است. در اين تحقيق بر آن شديم كه با استفاده از اين الگوي اعتبار بخشي دوره دستياري داخلي در يكي از دانشگاه هاي علوم پزشكي را بررسي نماييم. پژوهش حاضر با هدف تعيين ميزان تحقق استانداردهاي فدراسيون جهاني آموزش پزشكي (WFME) در دوره داخلي يكي از دانشگاه هاي علوم پزشكي تهران انجام شد. روش كار: مطالعه انجام شده در سال 1391 از نوع ارزشيابي كمي و كيفي همزمان است. حجم نمونه 4 نفر بوده كه به روش نمونه گيري در دسترس انجام شده است. ابزار جمع آوري اطلاعات چك ليست تهيه شده براساس استانداردهاي WFME در دو سطح پايه و كيفيت بود. اطلاعات با استفاده از تكميل چك ليست و مصاحبه، بررسي مستندات موجود و مشاهده فرآيندها و محيط هاي آموزشي، جمع آوري گرديد. پس از جمع آوري اطلاعات، داده هاي كمي وارد نرم افزار 16 SPSS گرديد و مصاحبه ها و مستندات و مشاهدات دسته بندي و مورد تحليل قرار گرفت و نتايج با استفاده از تجزيه تحليل داده هاي كمي و كيفي براي 9 حوزه گزارش گرديد. يافته ها: گروه داخلي در همه حوزه ها (9 حوزه رسالت، فرآيند آموزش، ارزيابي فراگيران، فراگيران، كاركنان، محيط و منابع آموزشي، ارزيابي فرآيند آموزشي، مديريت و امور اجرايي، نوسازي مداوم استاندارد پايه و در 6 حوزه؛ رسالت، فرآيند آموزش، ارزيابي فراگيران، كاركنان، ارزيابي فرآيند آموزشي، مديريت و امور اجرايي استاندارد كيفيت را را كسب نموده است و در حوزه هاي فراگيران، محيط و منابع آموزشي، نوسازي مداوم استاندارد كيفيت را كسب نكردند. نتيجه گيري: تسهيلات و امكانات لازم بخصوص در دروس عملي جزء نقايص است كه بايد برطرف شود (حوزه 6). گروه ها براي آموزش پزشكي مبتني بر شواهد هدف گذاري و برنامه ريزي مناسب انجام دهند. به نتايج ارزيابي ها اهميت بيشتري داده شود. مطالعات آينده نگر و جامعه نگر براي پويايي سازمان لازم است و بايد انجام شود (حوزه هاي 9، 1،6،7 ). پيشنهاد مي شود مطالعات بعدي در اين دانشگاه براي رفع نقايص موجود گروه هاي مورد بررسي انجام شود.

Introduction: In twenty-year vision document and the five-year developmental plan, adopted to promote the Iran global position, health and treatment are emphasized. One of the most valid international accreditation models in medical education is World Federation for Medical Education (WFME) accreditation model. The present study aims to evaluate the postgraduate training program Internal Medicine departments University of Medical Sciences (Tehran, Iran) according to the WFME accreditation model. The main objective of this study is the determining the establishment level of medical postgraduate education standards in the residency at Medical University according to the the World Federation of Medical Education (WFME) Methods: This was in 2012 a Quantitative and qualitative evaluation concurrent study. The sample size included 48 participants that were recruited using a convenience sampling. The WFME standards as a check list with two levels of base and quality was the instrument for data collection. The data was collected by completing the checklist and interview, reviewing existing documentation and observing the processes and educational environments. After collecting the data, quantitative data was entered into SPSS 16 software and the interviews, documentation and observations were categorized and analyzed and the results were reported using quantitative and qualitative data analysis for nine areas. Results: Internal Medicine departments achieved the basic standard criteria in all 9 fields and quality standard in 6 fields. However, they achieved no quality standard for 3 fields including Learners, educational resources and environment, continuous renovation. Conclusions: Necessary facilities especially for the practical courses are among shortcomings which have to be resolved (field 6). Department should make goal-oriented precise schedules based on evidences and evaluation results should be taken into consideration always, Community-based prospective studies are needed and should be done (fields; 1, 6, 7, 9). Further studies in order to remove present failures could be done.
