بررسي موارد و عوامل تاثيرگذار برخطاها در اتاق عمل: ديدگاه كاركنان

Factors affecting errors in the operating rooms: study of employees' viewpoint

سابقه و هدف: اتاق هاي عمل از پرخطرترين محيط ها به لحاظ امكان وقوع خطاهاي انساني مرتبط با سلامت هستند و كاهش خطا دراين محيط ها يك ضرورت براي بهبود كيفيت مراقبت سلامت است. هدف اين مطالعه، تعيين موارد خطا و عوامل تاثير گذار بر خطاها در اتاق عمل بود. روش بررسي: اين مطالعه توصيفي در سال 1395 روي 200 نفر از كاركنان اتاق هاي عمل بيمارستان هاي منتخب علوم پزشكي شهر تهران انجام شد. آنها به صورت تصادفي طبقه اي انتخاب شدند و و ديدگاه آنها با استفاده از پرسش نامه پژوهشگرساخته در سه بخش شامل مشخصات دموگرافيك، موارد خطا، و عوامل موثر در بروز خطا در چهار بعد فردي، محيطي، مهارتي و مديريتي بررسي شد. داده ها با آمار توصيفي - استنباطي و نرم افزار SPSS تحليل شدند. يافته ها: از ديدگاه كاركنان، بيمار را به اشتباه عمل كردن (71% )، و عضوي را به اشتباه عمل كردن (61%)، از موارد خطاهاي پر اهميت بود. پراهميت ترين عوامل موثر در بروز خطا، در بعد فردي، شرايط جسمي-رواني كاركنان (74%)، در بعد محيطي، تعداد زياد جراحي (69%)، در بعد مهارتي، ناآشنايي در استفاده از تجهيزات (5/60%)، و در بعد مديريتي، كم بودن تعداد كاركنان (74/5%) بودند. نتيجه گيري: برقراري ارتباط با بيماران، به كارگيري كاركنان ماهر و سالم از نظر جسمي-رواني، رعايت استاندارد به كارگيري كاركنان از نظر تعداد و تجربه، پرهيز از ازدحام و شلوغي، داشتن تجهيزات سالم و آشنايي با نحوه استفاده از آنها، مديريت صحيح موارد خطا و برخورد مناسب با خطاكار مي تواند ميزان خطا در اتاق هاي عمل را كاهش دهد.

Background: Operation rooms are one of the most hazardous environments in terms of possible human-related health hazards and reduction of errors in these environments is a necessity to improve the quality of health care. The purpose of this study was to determine the cases of errors and factors affecting the errors in the operating room. Materials and methods: This descriptive study was conducted in 2016, and the viewpoints of 200 employees of operating rooms in selected hospitals of Tehran Medical Sciences Universities were randomly categorized using a researcher-made questionnaire in three sections: demographic specification, error cases, effective factors in errors" in four dimensions of individual, environmental skill and managerial. Data were analyzed by descriptive-inferential statistics methods using SPSS software. Results: Regarding employees' viewpoint, wrong selection of a surgery (71%), and wrong surgery of an incorrect organ (61%) is the most important errors. The most important factors affecting the occurrence of faults in individual dimension was physical and mental condition of personnel (74%), in the environmental dimension the high number of surgical procedures (69%), in the skill dimension incomprehensibility in the use of equipment (60.5%), and in the management dimension low number of personnel in the surgical operation (74.5%). Conclusion: Communicating with patients, and engaging skilled and physically minded staff, observing the standards of staffing in terms of quantity and experience, avoiding crowds and crowding, having healthy and safe equipment and familiarity with how they are used, Proper management of error cases and appropriate conduct with the wrong doer can reduce the error rate in the operating rooms.
