Abstract :
PURPOE: To evaluate the relationhip between the circadian blood preure rhythm and the retrobulbar blood flow in glaucoma patient.
DEIGN: Cro-ectional tudy.
METHOD: Circadian blood preure meaurement and color Doppler imaging (CDI) in the ophthalmic artery a well a the central retinal artery of one randomly elected eye were obtained in 193 primary open-angle glaucoma patient. CDI parameter were compared by mean of analyi of covariance between patient with a nocturnal decreae in mean ytemic blood preure (MBP) below 20% of the average daytime MBP (over-dipper), patient with a decreae between 10% to 20% (dipper), and patient with a decreae of le than 10% (nondipper), uing age, intraocular preure (IOP), and MBP during color Doppler meaurement a covariate.
REULT: An analyi of covariance dicloed, after correcting for age, IOP, and MBP during color Doppler imaging, a ignificantly lower EDV (P = .0096) and a ignificantly higher RI (P = .033) in the central artery of over-dipping glaucoma patient compared with nondipper or dipper. Thi effect eemed independent of the ue of vaoactive drug.
CONCLUION: Glaucoma patient with a marked drop in nocturnal ytemic blood preure eem to have altered retrobulbar blood flow parameter, uggeting that an abnormal ytemic blood preure profile may be the manifetation of ome kind of ytemic vacular dyregulation relevant for the ocular circulation
