Retinal nerve fiber layer analyi: relationhip between optical coherence tomography and red-free photography

PURPOE: Comparion of retinal nerve fiber layer (RNFL) thickne meaurement uing optical coherence tomography (OCT) to the clinical tandard red-free photography (evaluated emiquantitatively), in relation to functional viual field damage in primary open-angle glaucoma (POAG) patient, ocular hypertenive, and POAG upect. METHOD: Concurrent, cro-ectional tudy. In four age-matched group (42 patient with early to moderate POAG, 34 ocular hypertenive, 22 POAG upect, and 25 normal control), RNFL wa aeed with OCT, tandardized red-free photographic coring method and tandard achromatic perimetry. REULT: OCT RNFL thickne decreae with increaed RNFL damage (detected with red-free photography). The global average OCT RNFL thickne correlated ignificantly with the photographic total RNFL core (r = .650, P = .0001). Both OCT and photographic coring ytem were able to find ignificantly thinner RNFL in the glaucoma group a compared with normal (P = .0001 for both), ocular hypertenive (P = .0001 for both), and upect (P = .0001 for both). However, neither OCT nor photography could ignificantly differentiate between ocular hypertenive, upect, and normal. Both OCT and photography were ignificantly correlated with VF lo. For the percentage of VF point depreed <5%, the correlation wa highet for OCT (r = −0.615 for OCT and r = −0.476 for photography). OCT had a higher diagnotic accuracy than photography (86% and 77%, repectively). CONCLUION: For RNFL thickne meaurement, the preence of high correlation between OCT, photographic core, and functional viual field lo ugget the validity of OCT meaurement. The higher diagnotic accuracy of OCT RNFL meaurement ugget it potential advantage for detection of early cae of glaucoma
