Abstract :
PURPOE: To evaluate the role of diffuion-weighted magnetic reonance imaging (DWIMRI) in the diagnoi and management of children with upected or confirmed haken Baby yndrome (B). METHOD: Thi wa a retropective interventional cae erie of all infant and children younger than 2 year of age admitted to a children’ hopital. We retropectively reviewed medical record and neuroimaging finding of all children younger than 2 year of age with confirmed or upected B admitted to a children’ hopital. Incluion criteria were documented ocular examination by an ophthalmologit and a brain MRI with DWI. Twenty-ix infant and children were included. Other children were excluded. Children with proven B were diagnoed with “confirmed B,” while children in whom the diagnoi of B remained uncertain were diagnoed with “upected B.” REULT: Twenty-ix infant and children with mean age of 7.1 month (range, 6 week–24 month) were included, 18 with confirmed B. All 26 patient had a ubdural hematoma, 10 had aociated occult bone fracture, and 18 had retinal hemorrhage. even of the eight cae without retinal hemorrhage had iolated ubdural hematoma without parenchymal brain leion on both conventional MRI and DWIMRI. B wa confirmed in only one cae with a normal fundu. Among the 18 patient with retinal hemorrhage, B wa confirmed in all but one cae. All 18 patient with confirmed B had an abnormal DWIMRI. In 13 patient, DWI howed leion that were larger than on conventional MRI. In patient with brain parenchymal leion, the DWIMRI characteritic uggeted cerebral ichemia, which appear to play a major role in B. CONCLUION: In all patient with confirmed B, DWIMRI wa abnormal and uggeted diffue or poterior cerebral ichemia, in addition to ubdural hematoma in the pathogenei of thi diorder.
