Abstract :
Purpoe To evaluate the efficacy of tranconjunctival Müller mucle receion and graded levator diinertion for eyelid retraction in patient with thyroid-related orbitopathy (TRO). Deign Retropective conecutive cae erie. Method Medical record review of 78 TRO patient (107 eyelid) who underwent urgery for upper eyelid retraction in a 5-year period wa performed. Main outcome meaure were anatomic and functional ucce, minimal reflex ditance (MRD), lagophthalmo, eyelid aymmetry, and patient dicomfort. Reult One hundred even eyelid retraction urgerie were performed on 78 TRO patient (63 women, mean age 49 year); mean follow-up time wa 16.7 month. Upper eyelid poition, lagophthalmo, expoure keratopathy, and patient’ dicomfort markedly improved after urgery (P < .001). Marginal reflex ditance (MRD1) decreaed an average of 2.6 mm from 6 mm pre-operatively to 3.4 mm pot-operatively (P < .001); lagophthalmo decreaed an average of 0.6 mm from 1.3 mm pre-operatively to 0.4 mm pot-operatively (P = .006) Failure rate wa 8.4%, mot improved with a econd urgery. Overcorrection wa noticed in three cae (2.8%). Eyelid aymmetry improved from a mean of 1.0 mm pre-operatively to 0.4 mm pot-operatively (P = .001); more than 80% of patient howed eyelid aymmetry of 1 mm or le. Concluion Tranconjunctival Müller mucle and levator receion i afe and effective in correction of mild, moderate, or evere eyelid retraction in TRO patient. The failure rate i le than 10% and may be addreed by a econd urgery.
