Abstract :
Purpoe To ae the effect of tiue-cultured human amniotic epithelial cell (AEC) in retoring the ocular urface, tranplanted uing a collagen hield eeded with AEC upported by a oft contact len. Deign Propective interventional ingle-intitutional cae erie with croover control. Method Three eye in three patient were identified with peritent corneal epithelial defect (PED) refractory to medical therapy. Two cae were econdary to neurotrophic keratopathy, while one cae wa attributable to longtanding alkali injury. AEC were iolated from erologically creened donor human placenta, eeded onto collagen corneal hield, and incubated in tiue culture medium for 7 day. Thee collagen hield were placed over the PED and upported by an overlying oft contact len. The collagen hield diolved by 72 hour, and the contact lene were removed after thi time. Thi cycle wa repeated every week until healing wa achieved. A a croover control, collagen hield without AEC were placed in the ame eye 1 week before placing collagen hield containing AEC. The PED wa aeed by vital taining and lit-lamp color photography. Reult The PED had a mean duration of 4 month and involved 20% to 37% of the corneal urface area, one cae econdary to longtanding alkali injury and two cae attributable to neurotrophic keratopathy. No change in PED ize wa oberved in thoe control eye receiving collagen hield without AEC. Complete reolution of the PED wa een after two cycle of AEC-eeded collagen hield in one cae, and four cycle in two cae, from 7 to 12 week following treatment in all patient. No lo of viual acuity wa een and clinical improvement wa maintained in all cae, with a mean follow-up of 6.3 month. Concluion Nonurgical tranplantation of tiue-cultured AEC on a collagen hield provide a promiing approach to retoring the ocular urface in cae of PED.
