Abstract :
Purpoe
To evaluate ue of a femtoecond laer to create haped corneal graft in penetrating keratoplaty (PK) in treatment of corneal dieae.
Deign
Propective, nonrandomized clinical tudy.
Method
ix eye of ix patient were treated in thi initial erie. Two had herpetic carring, two had peudophakic bullou keratopathy, one had Fuch endothelial dytrophy and one had endothelial dytrophy with a corneal car. Preoperative bet pectacle-corrected viual acuity (BCVA) ranged from count finger to 20/50. Donor and recipient cornea were cut in a “top-hat” haped configuration utilizing a 15-kHz femtoecond laer.
Reult
All eye were uccefully treated without intraoperative complication. Mean time to complete uture removal wa 7.0 ± 1.9 month (range, 4.1 to 9.7 month). At the one-year potoperative exam, mean endothelial cell denity wa 2030 ± 600 cell/mm2 (range, 1007 to 2497 cell/mm2), which wa comparable with cell denity meaured in a prior erie of traditional PK (P = .21). BCVA ranged from 20/400 to 20/25. In two eye, preexiting retinal condition limited viual improvement following tranplantation.
Concluion
Ue of the femtoecond laer allowed precie diection of donor and recipient cornea even when ignificant opacitie were preent. The ue of a top-hat configuration allowed for an increaed area of wound healing in thee eye.
