Viual Outcome and Bacterial enitivity After Methicillin-Reitant taphylococcu aureu–Aociated Acute Endophthalmiti Original Reearch Article

Purpoe To determine the frequency, viual outcome, and bacterial enitivity in cae of methicillin-reitant taphylococcu aureu (MRA)-aociated acute potoperative endophthalmiti occurring after cataract urgery. Deign Retropective, conecutive, obervational cae erie. Method ixty-four cae of acute endophthalmiti occurring within ix week after cataract urgery were identified over a three-year interval at a ingle vitreoretinal practice. Cae related to MRA were tudied and viual outcome and bacterial enitivitie were recorded. Reult Thirty-three of the 64 eye were culture poitive, and ix of the 33 cae (18.2%) demontrated MRA infection. All ix eye were tarted on fluoroquinolone antibiotic two or three day before urgery. Corneal abnormalitie were noted in five of the ix cae (83.3%), ranging from wound infection to diffue corneal opacification. Viual acuity at lat follow-up wa no light perception (NLP) in two eye, hand movement in two eye, and 20/30 or better in two eye. One eye with NLP viion underwent enucleation within three day of preentation becaue of panophthalmiti and impending epi. All ix organim were enitive in vitro to both gentamicin and vancomycin. No organim wa enitive to any fluoroquinolone antibiotic, although not all organim were teted againt all fluoroquinolone. Concluion MRA infection account for 18% of culture-poitive cae of endophthalmiti in thi tudy and wa aociated with a poor viual outcome in two-third of our patient. MRA organim may be reitant in vitro to all generation of fluoroquinolone antibiotic, but do eem to be enitive to gentamicin and vancomycin. The incidence of MRA endophthalmiti eem to have increaed ignificantly ince the Endophthalmiti Vitrectomy tudy wa publihed in 1996.
