Abstract :
Purpoe To evaluate the ue of the Boton keratoprothei (KPro) in patient with teven-Johnon yndrome (J). Deign Retropective, noncomparative, interventional cae erie. Method ixteen eye of 15 patient with J underwent KPro urgery at the Maachuett Eye and Ear Infirmary from January 2000 through December 2005. The preoperative, operative, and potoperative finding were recorded. All patient underwent either the type I or type II Boton KPro urgery by one urgeon (C.H.D.). Retention of the prothei, bet-corrected viual acuity, the need for urgical reviion, and potoperative complication were recorded. The outcome were compared with thoe of an earlier group of patient from the 1990. Reult The mean age of patient wa 50 ± 18 year (range, 23 to 74 year), and the mean duration of their dieae wa 10 ± 6.6 year. The mean follow-up period wa 3.6 ± 1.5 year (range, 10.2 month to 5.6 year). Ten eye underwent type II KPro urgery, wherea ix eye underwent type I KPro urgery. Twelve eye (75%) achieved a viual acuity of 20/200 or better after urgery, with eight eye (50%) achieving excellent viion of 20/40 or better. Viual acuity wa maintained at 20/200 or better over a mean period of 2.5 ± 2.0 year. Preexiting glaucoma wa found to be a ignificant rik factor for viual lo. There were no cae of KPro extruion or endophthalmiti. Concluion KPro in J ha improved, largely becaue of the introduction of vancomycin prophylaxi and better glaucoma treatment. It eem to be uperior to tandard penetrating keratoplaty, with or without allografted tem cell tranplantation, a judged from the literature. However, the outcome of the KPro in J i till ubtantially le favorable than in nonautoimmune dieae.
