Introduction: Road traffic injuries (RTIs) are considered as the most essential concern in public health and to be the second highest cause of mortality in Iran. The present study investigated the prevalence of RTIs leading to death in Mashhad, that is the second largest city in Iran. Method: In this descriptive and cross- sectional study, we conducted a study on the topic of traffic accidents-related deaths were referred to the Mashhad legal medicine organization, for a period of one-years, which started from March 2015 to March 2016 based on ICD-10 classification (v01–v99 codes). The main variables studied were deceased’s gender, age, nationality, status (i.e. driver, car occupant, pedestrians, etc.), in addition, cause, season and place of death. Result: From a total of 20611 individuals died in Mashhad in the study period, 857 (4 percent) from road traffic accidents.A disproportionate number of deceased individuals were male (76%), mostly aged 40 years or less (54%). 41% of them who were car occupants, 31.8% were pedestrians, 25.6% bicyclist and motorcycle riding and in 12%, the cause of the accident is unknown. 34% of deaths occur in summer. Conclusion: there is a pressing need to develop a comprehensive injury control policy and improve our recent strategies. In order to overcome this problem and reduce traffic-related mortality, these proceeding are recommended: Modification road traffic laws, patronizing people to use seat belts and helmets and obey the traffic laws, launching preventing road accidents campaigns, Strict monitoring on the issuing of driving permits, providing optimal road safety standards and transport infrastructure, holding training the general population in first aid and education courses for drivers and improving emergency healthcare services.
