Analysis of patients with secondary spontaneous pneumothorax
Abstract :
Aim: Secondary spontaneous pneumothorax (SSP) is collection of air in the pleural space due to underlying lung disease. In this trial, the etiological factors, clinical features, treatment, and the results of the SSP patients treated in our clinic were evaluated in light of pertinent literature.Materials and Methods: The data obtained from 74 patients who were treated for SSP in our clinic were analyzed retrospectively.Results: The average age of the patients was 57.72±13.83 (20-83) years; 68 were male, 6 female. The most frequent symptoms were shortness of breath (74%), chest pain (69%) and cough (12%). Chronic obstructive pulmonary disease (84%) was the most common etiological factor. Other factors were primary lung cancer (8%), metastatic lung disease (4%), interstitial lung disease (3%) and tuberculosis (1%). Twenty-four cases (32%) were operated on with video-assisted thoracoscopic surgery or open thoracotomy. The indications for surgery were prolonged air leakage (54%) and recurrence (46%). The other patients were treated with tube thoracostomy and pleurodesis (48%), only tube thoracostomy (42%), nasal oxygen inhalation therapy (8%) and thoracentesis (2%).Conclusion: Treatment of SSP is determined by the patient’s physical status, presence of underlying lung disease and recurrence. Patients could have poor pulmonary reserve due to advanced age and lung disease could be found upon admission. We believe that tube thoracostomy is an initial treatment for these patients because of increased operative risk. The addition of pleurodesis prevents the recurrence of disease.
