Factors Related with Duration of Hospital Stay in Patients with COPD

Abstract :
Objective: It is thought that hospitalization because of chronic obstructive lung disease (COPD) acute exacerbations is related with parameters that reflect the disease severity. Material and Method: We investigated retrospectively the demographic data and laboratory findings of patients who were treated in our clinic because of COPD acute exacerbation for 1 year, and the presence of variable parameters that can be related with hospital stay. Results: Of 113 cases inrolled into the study, 14 were female (12.4%) and 99 were male (87.6%). Median age of cases was 65.33±8.96. Duration of hospital stay was 16.02±12.85 day. Duration of hospital stay of ≥ 65 years old COPD patients was statistically shorter than 65 years old patients (p=0.047). Median duration of hospital stay was increased with increase in annual exacerbation number. Median duration of hospital stay of patients who have pulmonary hypertension, and history of intensive care unit (ICU) stay was more longer (p=0.024, p 0.001, respectively). A positive correlation was observed between duration of hospital stay and PaCO2, and pulmonary artery pressure (r=0.220, p=0.019, r=0.266, p=0.004, respectively), a negative correlation between oxygen saturation and FEV1 value (r= 0.327, p 0.001, r=-0.191, p=0.043, respectively). More than 1 exacerbation per year was an independent risk factor for ≥15 day hospital stay. Conclusion: It must be kept in mind that COPD patients who have more than 1 exacerbation per year, and history of ICU stay can have longer duration of hospitalization during exacerbation, so these patients must be warned.
