Wagner classification and culture analysis of diabetic foot infection

Abstract :
Objectives: The aim of this study was to determine the concordance ratio between microorganisms isolated from deep tissue culture and those from superficial culture in patients with diabetic foot according to Wagner’s wound classification method. Materials and methods: A total of 63 patients with Diabetic foot infection, who were admitted to Dicle University Hospital between October 2006 and November 2007, were included into the study. Wagner’s classification method was used for wound classification. For microbiologic studies superficial and deep tissue specimens were obtained from each patient, and were rapidly sent to laboratory for aerob and anaerob cultures. Microbiologic data were analyzed and interpreted in line with sensitivity and specifity formula. Results: Thirty-eight (60%) of the patients were in Wagner’s classification ≤2, while 25 (40%) patients were Wagner’s classification ≥3. According to our culture results, 66 (69%) Gr (+) and 30 (31%) Gr (-) microorganisms grew in Wagner classification ≤2 patients. While in Wagner classification ≥3; 25 (35%) Gr (+) and 46 (65%) Gr (-) microorganisms grew. Microorganisms grew in 89% of superficial cultures and 64% of the deep tissue cultures in patients with Wagner classification ≤2, while microorganism grew in 64% of Wagner classification ≥3. Conclusion: In ulcers of diabetic food infections, initial treatment should be started according to result of sterile superficial culture, but deep tissue culture should be taken, if unresponsive to initial treatment.
