Abstract :
Telehealth or eHealth can play an important motivational role in improving the patient´s/citizen´s physical condition and quality of life, and at the same time be an aid in monitoring a growing group of citizens with chronic diseases such as cardiovascular diseases and oncology. In this paper, we present an ICT architecture and key design requirements for a patient-centric ICT architecture that is based on an open-system approach where tablets and mobile phones can deliver the applications (Apps). The key elements of the ICT ecosystem architecture are a personal data store for every patient for medical data and Apps data, e-e trust assurance by data logging and the possibility to exploit Detailed Clinical Models for user-based healthcare monitoring and standardized pathways, agreed by both care professionals, experts and local practitioners. The new system will offer a uniform integrated platform for patient/citizens that can execute on mobile computing platforms with data storage in the cloud. The system is under development and validated for regional pilots with cardiovascular diseases.
