Abstract :
Failure Modes and Effects Analysis (FMEA) is a systematic technique of identifying, analyzing and preventing product and process problems before they occur. When implemented properly, FMEA´s is a valuable tool. When improperly executed, FMEA wastes time and resources and at its worst steers you in wrong directions. In this paper, we describe FMEA on FMEA, exploring how FMEAs go wrong and how to avoid these pitfalls.
