Abstract :
The Chronic Care Workstation (CCWS) has been developed for the rehabilitation and the follow-up of chronic patients. It facilitates access to the medical professionals-in particular in the field of rehabilitation-of support information that permits them to sustain better attention to and follow-up of their patients. The CCWS allows the management of patient data, the presentation of their clinical history, the elaboration of a patient´s agenda (activity modules) and a future care plan that involves the interaction of medical agents and social services. Furthermore, it makes possible the conference of generalized rehabilitation protocols and the customization of these protocols
