  • Abstract
    Caring for a patient requires informations from multiple source. Those information is contained is a medical record. Current implementation of medical record in developing countries left the medical record fractured and incomplete. This incompleteness creates gaps in medical practitioners knowledge of the patient´s conditions. This paper will describe the need of a complete medical record and a way to achieve it.
