Abstract
The absence of adverse performance trends or reported events does not necessarily indicate error-free performance. Low event rates or long duration intervals between significant events may actually be an indicator of performance issues coupled with lack of reporting of precursor level behaviors. It may be possible to identify “hidden trends” by comparing common causes from self-revealing events against those from observation and precursor level data.
