Abstract

This paper describes a project which consisted of revising the format of an existing electrical failure report form to correspond with the data collection format used in the 1973 IEEE Survey on Reliability of Electrical Equipment in Industrial Plants. The revised report form was then automated in the form of an electronic database. Revision of the report form is expected to increase the quality and quantity of the data submitted, and automation of the report form is expected to expedite the process of analyzing the data
