Abstract :
With the increased aging of the population, the prevalence rate of chronic diseases in elderly population increased year by year, the care of elderly with chronic diseases has become the focus of attention. This paper aims to the problems existing in the care of chronic disease among elderly in China, explores the value and significance of using the electronic health records to take care of elderly with chronic disease, and puts forward the idea of elderly chronic disease care based on electronic health records. It is believed that take full use of the electronic health records would strengthen prevention, treatment and care of elderly with chronic disease, promote the health of elderly comprehensively.
