Abstract
On February 4, 1999, the merchant ship New Carissa ran aground near Coos Bay, OR, USA. This study examines the various available reports, including material from the US Coast Guard, the Oregon Department of Environmental Quality, and the various federal agencies that were involved, in addition to news media and Internet material. The authors agree that a system was in place, and that attempts were made to follow the guidelines, but they cannot support the statement that it worked "reasonably well." Selected issues are analyzed to determine the cause of their failure, and the paper concludes with recommendations to improve a crisis project management plan
