Ethical Challenges in the Care of Emerging Diseases: A Systematic Literature Review

زمينه و هدف: مديريت بيماري‌هاي نوپديد، مستلزم رويكرد اخلاقي براي كنترل و مراقبت از اين بيماري‌ها است. مطالعه حاضر با هدف تعيين چالش‌هاي اخلاقي در مراقبت از بيماري‌هاي نوپديد انجام شده است. مواد و روش‌ها: در اين مرور نظام‌مند، با روش سه مرحله‌اي، مقالات منتشرشده به زبان انگليسي با استفاده از واژگان كليدي اصلي در پايگاه‌هاي اطلاعاتيScopus ،Pubmed ، Web of science از سال 1970 تا 2017 بازيابي شدند. سپس مقالاتي كه داراي معيارهاي ورود بودند، مورد مطالعه و تحليل قرار گرفتند. از 5833 مقاله بازيابي‌شده، 20 مورد مرور و تحليل شدند. براي استخراج داده‌ها، دو پژوهشگر با تجربه به طور هم‌زمان مرور و تحليل مقالات را انجام دادند، همچنين جهت افزايش اعتبار و پايايي مطالعه، دو نفر كيفيت مقالات را به طور جداگانه مورد ارزيابي قرار دادند. 16 مورد از مطالعات از نوع توصيفي (80‌%) و 4 مورد از نوع كيفي (20‌%) بودند. يافته‌ها: مهم‌ترين چالش‌هاي اخلاقي مطرح‌شده در مقالات شامل: رعايت حقوق بيمار، احترام به خودآييني بيمار، حفظ حريم خصوصي بيمار، اصل عمل متقابل، وظيفه مراقبت از بيمار، جلوگيري از انگ و تبعيض، مسؤوليت و پاسخگويي دولت‌ها بودند. نتيجه‌گيري: يافته‌ها نياز به در نظر قراردادن ملاحظات اخلاقي در مراقبت از بيماران مبتلا به بيماري‌هاي نوپديد را خاطرنشان مي‌سازد. گنجاندن آموزش اخلاق حرفه‌اي در مراقبت از بيماري‌هاي نوپديد و تصميم‌گيري اخلاقي در همه‌گيري‌ها، در برنامه درسي ارائه‌دهندگان مراقبت، جهت مديريت بهينه اين بيماري‌ها توصيه مي‌گردد.

Background and Aim: Management of emerging diseases requires an ethical approach to the control and care of these diseases. An ethical approach has been emphasized. The present study aimed to determine the ethical challenges in the care of emerging diseases. Materials and Methods: In this systematic review, using the three-step method, articles that published in English were retrieved using main keywords in the Scopus, PubMed and Web of Science databases from 1970 to 2017. Then, the articles with inclusion criteria were studied and analyzed. Out of the 5833 retrieved articles, 20 articles were reviewed. Analyzing the articles and extracting the data was done by two expired researchers simultaneously, as well as, evaluating the quality of the articles in order to increase the validity and reliability of the study. In terms of study type, 16 articles were descriptive (80%) and 4 cases (20%) were qualitative studies. Findings: The main ethical challenges identified included: patient rights, respect for patient autonomy, patient privacy, obligation of the patient care, reciprocity principle, accountability and responsibility of the states, and prevention of the patient stigmatization and discrimination. Conclusion: The findings highlights the need to consider ethical considerations in planning and taking care for emerging diseases. The inclusion of professional ethics training in the management of emerging diseases and ethical decision-making in epidemics in the curriculum of health care providers is recommended for better management of these diseases.
