Evaluation heart rate irritability in children with chronic medical conditions based on attachment style

زمينه: روانشناسي سلامت كودك و نوجوان و بررسي نقش عوامل روان‌شناختي در چگونگي سازگاري كودكان با شرايط پزشكي، از حوزه‌هاي قابل توجه در روانشناسي سلامت است. يكي از اين عوامل روان ­شناختي كه در پژوهش‌هاي متعدد نقش مهمي را در سلامت به خود اختصاص داده سبك دلبستگي است. هدف: پژوهش حاضر با هدف سنجش تحريك‌پذيري ضربان قلب كودكان مبتلا به بيماري مزمن پزشكي بر اساس سبك دلبستگي انجام گرفت. روش: روش اين پژوهش از نوع شبه آزمايشي است، به اين منظور 45 كودك 15-12 ساله واجد ملاك­ هاي طرح، كه با تشخيص يك بيماري مزمن تحت درمان پزشكي بودند، پس از اعلام موافقت وارد طرح پژوهش شدند. كليه كودكان با تشخيص يك بيماري مزمن پزشكي تحت درمان پزشكي بودند. اين كودكان مبتلا به بيماري هاي ديابت، نارسايي كليوي و آسم بودند. پس از تكميل پرسشنامه سبك دلبستگي كولينز و ريد(1990) توسط اين كودكان، آنها بر اساس نمرات اخذ شده در سه گروه با سبك دلبستگي ايمن، اجتنابي و اضطرابي قرار گرفتند. در گام بعد نيز سنجش تحريك‌پذيري ضربان قلب انجام شد. يافته ­ها: يافته‌هاي پژوهش نشان‌ داد در كودكان با سبك دلبستگي ايمن، ميزان معناداري 0/05p< در نمرات دلبستگي و زير مقياس‌هاي تحريك‌پذيري ضربان قلب شامل نرخ ضربان قلب و نرخ تنفس در مقايسه با گروه‌هاي با سبك دلبستگي ناايمن ديده شد. نتيجه ­گيري: مطالعه حاضر نشان مي­ دهد كه ابتلا به بيماري ­هاي مزمن پزشكي كودكان مي ­تواند منجر به عواقب رواني- اجتماعي مهمي در آنها و والدينشان گردد و ميزان سازگاري آنها را تحت تأثير قرار دهد و سبك دلبستگي كودكان مي ­تواند در اين ميان نقش مؤثري داشته باشد.

Background: Pediatric health psychology and investigating the role of psychological factors in adaptation of children with medical conditions is a notable field in health psychology. One of these psychological factors is the attachment style which plays an important role in the literature. Aims: The present study was conducted with the aim of evaluating the heart rate irritability in the children with chronic medical conditions based on attachment style. Method: In this quasi-experimental study, the effect of attachment psychological factor on heart rate irritability in children with chronic medical conditions was investigated this end, 45 children aged 12-15 years with the criteria for diagnosis of a chronic medical condition were selected by sampling method. These children were suffering from diabetes and renal failure and nasal congestion. entered the study After completing the Collins and Reid (1990) Attachment Style Questionnaire, children were divided into three groups with security, avoidant and anxiety attachment style. In the next step, heart rate irritability and measurements were performed. Results: Finding of the study showed that in children with security attachment style, there was a significant difference p<%05 in attachment scores and heart rate excitatory subtypes including heart rate and respiration rates as compared to those with insecure attachment style. Conclusions: This study showed that the chronic medicalconditions in children lead to psychosocial harm in them and their parents and their compatibility is affected, and the attachment style of children can affect this relationship.
