ﺳﺎﺑﻘﻪ وﻫﺪف: ﺳﻄﺢ ﺳﺮﻣﯽ آﻟﻔﺎﻓﯿﺘﻮﭘﺮوﺗﺌﯿﻦ )FP( در ﺑﺴﯿﺎري از ﺗﻮﻣﻮرﻫﺎي ﻣﺎدرزادي ﻣﺜﻞ ژرم ﺳﻞ، ﻫﭙﺎﺗﻮﺑﻼﺳﺘﻮم و ﻫﻤﭽﻨﯿﻦ ﺑﯿﻤﺎريﻫﺎي ﮐﺒﺪي و ﻣﺘﺎﺑﻮﻟﯿﮏ در ﻧﻮزادان ﺑﻌﻨﻮان ﯾﮏ ﻣﺎرﮐﺮ ﺗﺸﺨﯿﺼﯽ در ﺣﺪ ﺑﺎﻻﺗﺮ از ﻧﺮﻣﺎل ﻣﯽﺑﺎﺷﺪ. ﻧﻮزادان ﻧﺮﻣﺎل ﻧﯿﺰ ﺳﻄﺢ ﺳﺮﻣﯽ آﻟﻔﺎﻓﯿﺘﻮﭘﺮوﺗﺌﯿﻦ ﺑﺎﻻﺗﺮ از دﯾﮕﺮ ﺳﻨﯿﻦ زﻧﺪﮔﯽ دارﻧﺪ ﻟﺬا در ﺗﺸﺨﯿﺺ اﻓﺘﺮاﻗﯽ اﯾﻦ ﺗﺪاﺧﻞ ﻣﻬﻢ اﺳﺖ. در ﻣﻨﺎﺑﻊ ﻣﻌﺘﺒﺮ، ﻣﻌﯿﺎر ﺳﺮم آﻟﻔﺎﻓﯿﺘﻮﭘﺮوﺗﺌﯿﻦ ﻧﺮﻣﺎل در ﻧﻮزادان ﻣﺮﺑﻮط ﺑﻪ ﮐﺸﻮرﻫﺎي ﭘﯿﺸﺮﻓﺘﻪ اﺳﺖ ﮐﻪ ﻣﻤﮑﻦ اﺳﺖ در ﮐﺸﻮر ﻣﺎ ﻣﺘﻔﺎوت ﺑﺎﺷﺪ. ﻟﺬا اﯾﻦ ﻣﻄﺎﻟﻌﻪ ﺑﻪ ﻣﻨﻈﻮر ﺗﻌﯿﯿﻦ ﻣﻘﺎدﯾﺮ ﺳﺮﻣﯽ آﻟﻔﺎ ﻓﯿﺘﻮﭘﺮوﺗﺌﯿﻦ در ﺧﻮن ﺑﻨﺪﻧﺎف ﻧﻮزادان ﺗﺮم ﻣﺘﻮﻟﺪ ﺷﺪه در ﺑﺎﺑﻞ و ﻣﻘﺎﯾﺴﻪ آن در دو ﺟﻨﺲ اﻧﺠﺎم ﺷﺪ. ﻣﻮاد و روشﻫﺎ: اﯾﻦ ﻣﻄﺎﻟﻌﻪ ﻣﻘﻄﻌﯽ ﺑﺮ روي 500 ﻧﻮزاد ﺗﺮم )37-42 ﻫﻔﺘﻪ( ﻣﺘﻮﻟﺪ ﺷﺪه در ﺑﯿﻤﺎرﺳﺘﺎنﻫﺎي ﺷﻬﺮﺳﺘﺎن ﺑﺎﺑﻞ ﮐﻪ ﻣﻌﺎﯾﻨﻪ ﻓﯿﺰﯾﮑﯽ آﻧﻬﺎ ﻧﺮﻣﺎل ﺑﻮد، اﻧﺠﺎم ﺷﺪ. در ﺑﺪو ﺗﻮﻟﺪ اﻧﺘﺨﺎب 5 ﻣﯿﻠﯽ ﻟﯿﺘﺮ ﺧﻮن ﺑﻨﺪﻧﺎف از آﻧﻬﺎ ﮔﺮﻓﺘﻪ ﺷﺪ و ﻧﻤﻮﻧﻪ ﻫﺎ ﺟﻬﺖ اﻧﺪازهﮔﯿﺮي آﻟﻔﺎﻓﯿﺘﻮﭘﺮوﺗﺌﯿﻦ ﺑﻪ آزﻣﺎﯾﺸﮕﺎه ارﺳﺎل ﮔﺮدﯾﺪ. ﺳﻄﺢ ﺳﺮﻣﯽ آﻟﻔﺎﻓﯿﺘﻮﭘﺮوﺗﺌﯿﻦ ﺑﻪ روش اﻟﯿﺰا اﻧﺪازهﮔﯿﺮي و در دو ﺟﻨﺲ ﻣﻮرد ﻣﻘﺎﯾﺴﻪ ﻗﺮار ﮔﺮﻓﺖ. ﯾﺎﻓﺘﻪﻫﺎ: ﻣﯿﺎﻧﮕﯿﻦ ﺳﻄﺢ ﺳﺮﻣﯽ آﻟﻔﺎﻓﯿﺘﻮﭘﺮوﺗﺌﯿﻦ ng/ml 76571/20±35248/32 )ﺑﯿﻦ ng/ml 2300-160000( ﺑﻮد. ﻣﯿﺎﻧﮕﯿﻦ در ﭘﺴﺮان ﺑﯿﺸﺘﺮ از دﺧﺘﺮان ﺑﻮد )80548/81±36950/33ng/ml در ﻣﻘﺎﺑﻞ ng/ml 73690/86±33735/06( ﮐﻪ اﺧﺘﻼف آﻣﺎري ﻣﻌﻨﯽدار ﺑﻮد )0/002=p(.
ﻧﺘﯿﺠﻪﮔﯿﺮي: ﻧﺘﺎﯾﺞ اﯾﻦ ﻣﻄﺎﻟﻌﻪ ﻧﺸﺎن داد ﮐﻪ ﺳﻄﺢ ﺳﺮﻣﯽ آﻟﻔﺎﻓﯿﺘﻮﭘﺮوﺗﺌﯿﻦ در ﻧﻮزادان ﻣﺘﻮﻟﺪ ﺷﺪه در ﺷﻬﺮ ﺑﺎﺑﻞ ﻧﺴﺒﺘﺎ ﺑﺎﻻ ﻣﯽ ﺑﺎﺷﺪ و ﻫﻤﭽﻨﯿﻦ در ﭘﺴﺮان ﺑﯿﺸﺘﺮ از دﺧﺘﺮان ﻣﯽ ﺑﺎﺷﺪ
چكيده لاتين :
Serum alpha-fetoprotein (αFP) level is considered as a diagnostic marker is higher than normal in many congenital tumors such as germ cell, hepatoblastoma, as well as liver and metabolic diseases in neonates. Normal neonates also have a higher level of alpha-fetoprotein than others, so it is important to diagnose this interference. In valid sources, the normal serum alpha-fetoprotein level in infants is related to advanced countries, which may vary in our country. Therefore, this study was conducted to determine the serum levels of alpha-fetoprotein in the umbilical cord blood of term neonates born in Babol and to compare them in two genders.
METHODS
This cross-sectional study was performed on 500 neonates (37-42 weeks) born in hospitals in Babol city where physical examination was normal. At birth, 5 ml of umbilical cord blood was taken and samples were sent to the lab for measurement of alpha-fetoprotein. Serum alpha-fetoprotein level was measured by ELISA method and was compared in two genders.
FINDINGS
Mean serum a FP levels was 76.57±35.25 ng/ml (range 2.3-160) and it was significantly higher in males (80.54±36.95 vs. 73.69±33.73 ng/ml) which was statistically significant (p=0.002).
CONCLUSION
The results of this study showed that the level of alpha-fetoprotein in neonates born in Babol is relatively high and also in males is more than females.
