Ethics in precision medicine

پزشكي دقيق علم جديدي است كه به‌دنبال استفاده از اطلاعات دقيق و وسيع در مورد تنوع فردي به منظور بهبود در پيشگيري، تشخيص و درمان با استفاده از دارو ايجاد شده است. هدف از اين مطالعه مروري بررسي ابعاد مختلفي از چالش‌هاي اخلاقي مرتبط با اين علم است. در اين مطالعه مروري نظام‌مند پس از جستجو در پايگاه‌هاي اطلاعاتي مانندPubMed ، ISI و Scopus با بررسي هدفمند متون در رابطه با اخلاق در پزشكي دقيق، در مجموع 47 مقاله جمع آوري گرديد كه از ميان آن‌ها 38 مقاله انتخاب و مورد بررسي قرار گرفت. از بررسي مجموع مطالعاتي كه معيار ورود به مطالعه حاضر را داشتند، اين مهم بر مي‌آيد كه برخلاف اينكه در نگاه اول به نظر مي‌رسد تحقق اهداف شگفت‌انگيز پزشكي دقيق و رشد آن نيازمند سرمايه‌گذاري‌هاي كلان در بلند مدت دارد و با چالش-هاي اقتصادي رو‌به‌روست، ولي موانع ترويج پزشكي دقيق چالش‌هاي فرهنگي است. تنها راه حل، تلاش طرفداران اين علم براي درك و پذيرش موانع اخلاقي، اقتصادي و سياسي به منظور تغيير فرهنگ براي ارتقا پزشكي دقيق است. بررسي‌ها نشان مي‌دهد كه پزشكي دقيق درصورتيكه در چهارچوب اخلاقيات چه در زمينه پژوهش و چه در زمينه تشخيص و درمان موثر افراد به پيش رود، مي‌تواند دريچه‌اي نوين از مراقبت‌هاي بهداشتي با عملكردي بسيار بهينه‌تر از گذشته ايجاد كند. به طوريكه مي-توان انتظار داشت در آينده‌اي نه چندان دور شاهد پيشرفت سريع اين علم هم در حوزه تحقيقات و هم در حوزه بالين باشيم.

Precision medicine is a new science that applies a broad and accurate order of information on individual diversity to improve prevention, diagnosis, and treatment by medication. The aim of this review study was to examine the various dimensions of the ethical challenges associated with this science. In this study after searching in databases such as PubMed, ISI and Scopus, 47 articles were obtained by exploring the purposeful texts on ethics in precision medicine, among which 38 articles were selected for review. From the survey of the total number of studies that were the criteria for entering this study, it is important that despite the fact that at first glance, the realization of the amazing goals of precision medicine and its growth requires large investments in the long run and with the economic challenges but the obstacles to the promotion of medical precision are cultural challenges. The only solution to the efforts of the advocates of this science is to understand and accept the ethical, economic, and political barriers to changing the culture for precision medicine upgrading. Studies show that precision medicine can provide a healthier alternative to healthcare with much better performance than it used to be, in the ethical framework, both in research and in the field of effective diagnosis and treatment. In the near future, we can expect rapid progress in science both in the field of research and in the clinical field.
