بررسي ميزان همسبتگي و دقت نرم‌افزارهايPlanmeca Romexis و efilm در برآورد دانسيته استخوان محل قرارگيري ايمپلنت

Evaluation of Planmeca Romexis and efilm softwares in assessing the trabecular bone density at dental implant sites using cone-beam computerized tomography(CBCT)

خلاصه: سابقه و هدف: موفقيت درمانهاي ايمپلنت تحت تاثير عوامل متعددي از جمله حجم استخوان و دانسيته استخوان در دسترس براي قرارگيري ايمپلنت مي‌باشد. استفاده از نرم افزارهاي كامپيوتري جهت توصيف مقياس مشاهده اي از دانسيته استخوان پيشنهاد گرديده است. اين مطالعه با هدف تعيين ميزان دقت نرم افزارهاي efilm و Planmeca Romexis در برآورد دانسيته استخوان محل قرار گيري ايمپلنت و عوامل مرتبط با آن، در مراجعين به كلينيك خصوصي،‌ در سال 1387، انجام گرفت. مواد و روش‌ها:‌ در اين مطالعه توصيفي تصاوير CT شامل 42 ناحيه ماگزيلاري و 41 ناحيه منديبولار مربوط به 83 ناحيه قرارگيري ايمپلنت مورد بررسي قرار گرفت. دانسيته استخوان ترابكولار با دو نرم افزار efilm و Planmeca Romexis اندازه گيري شد. داده‌ها استخراج و با استفاده از آزمون هاي Paired T-test و Multiple linear regression test(روش Backward LR) تحت قضاوت آماري قرار گرفت. يافته‌ها: ميزان همبستگي دانسيته استخوان با هر دو روش برابر 0.86 و يا عالي بود. ميزان اختلاف در ناحيه قدام مندبيل در دو روش برابر 108 + 56.8 و خلف مندبيل برابر 105 + 57.9 و قدام ماگزيلا برابر 64 + 105 و بالاخره براي خلف ماگزيلا برابر 72 + 57.4 بود(p < 0.05). نتيجه گيري:‌ به نظر مي‌رسد كه نرم‌افزارها در برآورد دانسيته استخوان مشابه با هم باشند و برآورد دانسيته استخوان در نواحي مختلف جايگذاري ايمپلنت متفاوت است.

Abstract: Background and Aim: successful dental implant therapy is influenced by many factors such as volume density of the bone. The use of computer softwares for evaluating the bone density in Hounsfield Unit has been recommended. The aim of this study was to assess the bone density and related factors in patients requiring implant therapy, using efilm and Planmeca Romexis softwares. Materials and Methods: in this descriptive study, CT images of 42 maxillary and 41 mandibular implant sites were studied. Trabecular bone density was assessed by efilm and Planmeca Romexis softwares. Data analysis was performed by Paired T-test and Multiple linear regression test (Backward LR). Results: strong correlation was found between two softwares(r=0.856). Mean bone density with both softwares was higher in anterior mandilble, followed by posterior mandible, anterior maxilla & posterior maxilla. The differences between the regions was highly significant (P < 0.001).Among all the related factors there was only a statistically significant association between the Hounsfield values and the anatomical region. Conclusion: This study confirms the impotrtance of a site specific bone tissue evaluation prior to implant installation. A quantitive reliable scale could be helpful if it is taken together with all clinical relevant information for implant treatment planning.
