الگوي اختلالات زباني در بيماران آسيب ديده مغزي: شواهدي از بيماران دوزبانه كردي- فارسي

The Pattern of Language Deficit in Aphasic Patients who Suffer from CVA: Evidences of Kurdish-Persian Bilinguals

چكيده مقدمه: اختلالات زباني اعم از توليدي و دركي در وجوه متفاوت زبان، در بيماران آسيب ديده مغزي به چشم مي خورد. بررسي و تحليل اين نوع اختلالات مي تواند در درمان بهتر و سريع تر اين بيماران موثر واقع شود. اين مطالعه با هدف بررسي و تعيين نوع آفازي در بيماران دو زبانه كردي- فارسي آسيب ديده مغزي(CVA) و بررسي الگوي بازگشت زباني در دو زبان صورت گرفته است. مواد و روش ها: در اين مطالعه به منظور بررسي و تعيين نوع اختلالات زباني دربيماران آفازي دو زبانه كردي-فارسي، آزمون زبان پريشي افراد دو زبانه(BAT) كه در دو گونه فارسي و كردي تنظيم شده است، مورد استفاده واقع شده است. آزمودني هاي اين پژوهش شامل سه بيمار آفازي دو زبانه كردي-فارسي مي باشند كه كردي زبان اول و فارسي زبان دوم آن ها بوده است كه به علت(CVA) و آسيب در نيمكره چپ مغز مدتي در بيمارستان بستري بوده اند. در اين پژوهش از سه نفر فرد سالم و طبيعي كه سابقه هيچ گونه اختلال رواني و يا آسيب مغزي نيز نداشته اند، به عنوان گروه كنترل استفاده شده و نتايج و داده هاي حاصل از عملكرد اين دو گروه مورد بررسي مطالعه و ارزيابي قرار گرفتند. يافته هاي پژوهش: بر اساس يافته هاي حاصل از اين پژوهش بيماران بيشترين اختلال زباني را در حوزه توليد زباني و تكرار داشتند. توليد اين بيماران تلگرافي، شامل كلمات محتوايي بود و تكواژهاي نقشي در گفتار آن ها اغلب محذوف بود، توانايي تكرار هم در اين بيماران بسيار ضعيف بود، در حدي كه قادر به تكرار هيچ جمله ا ي نبودند، برعكس قابليت درك زباني در آن ها بسيار خوب و بالاتر از حد متوسط بود. نيمرخ زباني اين بيماران در هر دو زبان كردي و فارسي تقريباً يكسان بوده و الگوي اختلالات و بازگشت زباني در هر دو زبان تفاوت چنداني با هم نداشته اند. بحث و نتيجه گيري: نوع اختلال آفازي در بيماران دو زبانه كردي-فارسي كه در نيمكره چپ دچار آسيب بودند بر اساس داده هاي حاصل از آزمون زبان پريشي افراد دو زبانه در هر دو زبان بروكا تشخيص داده شد و الگوي بازگشت زباني نيز منطبق بر الگوي موازي بوده است.

Abstract Introduction: Production and compreh-ension disorders, in various aspects of lang-uage, are common conditions that have been observed among the patient who suff-ered from CVA. Analysis of these impair-ments would be effective in their rapid and better treatment. The aim of this study was to determine the type of aphasia and recov-ery pattern of Kurdish- Persian bilingual aphasic patients. Materials & methods: In this study, Kur-dish and Persian versions of bilingual aph-asic test (BAT) have been used in order to investigate the type of aphasia and language impairment in three Kurdish- Persian bilin-gual aphasic patients. They were in hospital because of CVA and damage in left hemi-sphere. Three other persons without any brain damage or psychological problems were randomly chosen as control group. Their language performance were compared and analyzed in a systematic way. Findings: Data indicated that the aphasic patients have more difficulty and impair-ment in language production and repetition rather than comprehension; their speech were characterized by a difficulty with function words and inflectional morphemes, their language production were so sim-plified, paraphasic and telegraphic which contained the lexical words and almost always omitted functional words. Their repetition ability was so weak that they were not able to repeat no sentence corre-ctly. On the other hand, their speech com-prehensions were good and satisfactory. Their language profile for Persian and Kur-dish were alike, and language deficit and recovery pattern of these two languages showed no significant differences and somehow they were the same. Discussion & conclusion: The results sho-wed that the type of aphasia of these three bilingual aphasic patients whose left hem-isphere were damaged is Broca area, in both Kurdish and Persian and the recovery patte-rn was accord to a parallel model.
