بررسي تاثير آموزش با رويكرد مشاركتي بر خودكارآمدي و خودباوري مراقبين خانوادگي بيماران تحت باي پس عروق كرونر

The Effect of Education with the partnership approach on the self-efficacy and self-esteem of family caregivers of patients undergoing Coronary Artery Bypass Graft (CABG)

زمينه و هدف: مراقبت از بيماران تحت جراحي باي پس كرونر (CABG) نگراني‌هاي زيادي براي مراقبين خانوادگي ايجاد مي‌كند. ارتقا توانايي‌ مراقبت كنندگان به‌عنوان همراهان هميشگي بيمار، امري ضروري است. مطالعه حاضر با هدف تعيين تاثير آموزش با رويكرد مشاركتي بر خودكارآمدي و خودباوري مراقبين خانوادگي بيماران تحت باي پس عروق كرونر انجام گرفت. مواد و روش‌ها: در كارآزمايي باليني حاضر، 102 مراقبت كننده از بيماران بستري در بخش جراحي قلب يكي از بيمارستان‌هاي شهر تهران در سال 1390، به روش در دسترس انتخاب و به صورت تصادفي در دو گروه 51 نفره مداخله و كنترل قرار گرفتند. براي گروه مداخله 5 جلسه برنامه آموزشي با رويكرد مشاركتي اجرا گرديد. قبل و 8 هفته پس از اجراي مداخله، خودكارآمدي و خودباوري مراقبين با استفاده از پرسشنامه‌هاي پژوهشگر ساخته اندازه‌گيري شد. داده‌ها با استفاده از آزمون‌هاي آماري تي‌مستقل، تي‌زوج، كاي دو و آزمون دقيق فيشر تحت نرم افزار 18 PASW مورد تجزيه و تحليل قرار گرفتند. يافته‌ها: در گروه مداخله ميانگين نمرات خودكارآمدي مراقبين پس از مداخله از 89/3 ± 49/29 به 93/1 ± 43/47 و ميانگين نمرات خودباوري از 03/6 ± 49/93 به 52/18 ± 13/126 افزايش پيدا كرد (001/0 > P). همچنين نتايج آزمون آماري تي مستقل نشان داد خودكارآمدي (001/0 > P) و خودباوري (002/0 = P) مراقبين بين دو گروه مداخله و كنترل تفاوت آماري معني داري داشته است. نتيجه‌گيري: آموزش مراقبين خانوادگي بيماران تحت جراحي باي پس كرونر با رويكرد مشاركتي مي‌تواند منجر به ارتقاي خودكارآمدي و خودباوري و در نتيجه افزايش مشاركت آن‌ها در ارايه مراقبت كارآمد شود. به‌ كارگيري اين روش در فرايند آموزش پيشنهاد مي‌گردد.

Background and Objective: Care of patients undergoing coronary artery bypass graft (CABG) creates some concerns for family caregivers. Increasing the ability of caregivers as accompanies the patient is essential.The purpose of current study was to determine the effect of education with the partnership approach on the self-efficacy and self-esteem of family caregivers of patients undergoing coronary artery bypass graft. Materials and Method: In this clinical trial study 102 caregivers of the hospitalized patients in cardiac surgery ward in one Hospital in Tehran, 2011 were selected through convenience sampling and randomly allocated into two intervention and control groups of 51 people. For intervention group, 5 sessions educational program with partnership approach were conducted. Self-efficacy and self-esteem of caregivers were measured before and 8 weeks after intervention through using the researcher-made questionnaires. Data analysis was done through PASW 18 Statistics and using independent and paired t-test, chi-square and Fisher exact test. Results: In intervention group, the mean score of caregiver’s self-efficacy was significantly increased from (29.49 ± 3.89) to (47.43 ± 1.93) (p < 0.001). As well as the mean score of caregiver’s self-esteem was increased from (93.49 ± 6.03) to (126.13 ± 18.52) (p < 0.001). Also, the results of independent t-test showed the significant difference concerning the self-efficacy (p < 0.001) and self-esteem (p < 0.002) between two groups. Conclusion: Education of family caregivers of patients undergoing coronary artery bypass graft with a partnership approach can lead to improvement the caregivers’ self-efficacy and self-esteem and thus the increasing of their participation in efficient care.
